Abstract


Objective:1 Compare the level of IL-1βin blood serum between COPD group and control.2 To determine the effect of polymorphism of IL-1β(-511、-31、+3954) on the affectability of chronic obstructive pulmonary disease(COPD).Methods:A selection to subjects of COPD was performed in KaiLuan hospital begin with February 2007. At same time, the controls were recuited in medical examine centre. The proportion of case and control was 1:1. The control of matching must be same gender to caseand the difference of ages can not bigger then 5. The numbers of COPD and control group were 162 (male 126/female 36) respectively. The data was obtained by the physical examination and a unified questionnaire. 4ml fasting blood samples were kept. The contents of questionnaire included basic information (age, sex, height, weight, waist, blood pressure and heart rate), marital status, level of education, occupation, past medical history, tobacco use, alcohol use, medication history and family history. The blood samples were used to measure the level of IL-1βin serum and to measure the gene frequency of the three sites (-511, -31, +3954). Enzyme-linked immunosorbent assay (ELISA) was used to measure the level of IL-1βin blood serum. Polymerize chain reaction-Restriction fragment length polymorphism (PCR-RLFP) was used to detect the genotypes of IL-1β(-511, -31, +3954). Multivariate logistic regression was used to explore the relations between risk factors with susceptibility of COPD. The data was analyzed by SPSS13.0 statistics package. A P value<0.05 was considered significant.Results: The level of IL-1βin blood serum was 3.96±0.35 ng/ml in COPD group, the concentration was 2.69±0.14 in control group, no significance was found (P<0.0001). In male, the concentration of IL-1βwere 4.02±0.47 and 2.73±0.22 ng/ml in COPD and control group, no significance was found (P<0.0001). In female, the concentration of IL-1βwere 3.84±0.43 and 2.65±0.11 ng/ml in COPD and control group, no significance was found (P<0.0001).The frequency of genotype of -511 site were: CC-32(20%), CT- 102(63%) and TT-28(17%) in COPD group and CC-36(22%), CT-99(61%) and TT-27(17%) in control group. The frequency of genotype of -31 site were: CC-32(20%), CT- 101(62%) and TT-29(18%) in COPD group and CC-36(22%), CT-99(61%) and TT-27(17%) in control group. The frequency of genotype of -31 site were: CC-141(87%), CT-21(13%) and TT-0(0%) in COPD group and CC-146(90%), CT-15(9%) and TT-1(1%) in control group (All P<0.05).In total, the frequency of allele T of -511 site were 48.77% and 47.22% in COPD group and control group, the frequency of allele C of -511 site were 51.23% and 52.78% in COPD group and control group, no significant difference was found (P=0.694). The frequency of allele T of +3954 site were 6.48% and 5.25% in COPD group and control group, the frequency of allele C of +3954 site were 93.52% and 94.75% in COPD group and control group, no significance difference was found (P=0.637). The frequency of allele T of -31 site were 49.07% and 47.22% in COPD group and control group, the frequency of allele C of -31 site were 50.93% and 52.78% in COPD group and control group, no significance difference was found (P=0.504). Not only in total but also in subgroup of men and women, the differences of allele frequency in three sites were not found. But the difference of combine frequency of allele in -31 site and +3954 site was found between COPD group and control (P=0.034).The results of multivariate Logistic regression showed that smoking, depression, with family history and combine frequency of allele in -31 and +3954 sites could increase the risk of COPD. The risk of COPD in person with family history was 1.162 times than person not with family history. The 95% confidence interval (CI) was 1.076 to 1.267. Smoking can increase the risk of COPD and the 95%CI was 1.359(1.173, 1.574). The risk of COPD in person who was depression was 1.274 times than person who was not depression; the 95%CI was 1.092 to 1.487. The combine frequency of allele in -31 and +3954 sites can increase the risk of COPD and the 95%CI was 1.033(1.025, 1.042).Conclusion:1. The consentration of IL-1βin serum in COPD group was higher than in control group.2. The polymorphism of -511, -31 and +3954 have no relations with COPD.The Combine frequency of -31 and +3954 can increased the risk of COPD.
