Objective Through the implementation of the medical staff hand hygiene and hand carrier status of research to understand the current situation of medical staff hand hygiene and hand hygiene knowledge to grasp the situation and the impact of hand hygiene compliance factors, aimed at improving the medical staff working in clinical The hand-washing awareness; explore the medical staff can improve hand hygiene, improve hand-washing compliance and interventions to reduce hospital infection rates for hospital infection prevention, control and management to provide a scientific basis. Method uses a combination of qualitative and quantitative research methods, Tai'an City, a first in the three-level general hospitals, including internal medicine, surgery, intensive care unit, emergency department and other clinical departments randomly selected 180 medical staff conducted baseline surveys as research subjects , including 87 doctors, 93 nurses, using observation and understanding of the study were collected by questionnaire handwashing behavior, perform hand hygiene compliance in hand hygiene factors and mastery of knowledge and nurses wash hands after sampling the sample specimens for bacterial culture to determine wash pass rate. For medical staff hand hygiene status and influence factors of hand hygiene compliance, to develop interventions, choose two basic conditions for the same ward of the medical staff of 24 as the research object controlled trial to evaluate the effect before and after intervention. Results 1. Medical staff hand hygiene situations: surveyed 180 medical staff hand hygiene pass rate of 56.67%; hand hygiene compliance rate averaged 29.79 ± 15.21. Different ages, education, job title, gender, occupation of the medical staff hand hygiene compliance rate was not significantly different (p gt; 0.05), critical care ward medical staff hand hygiene compliance rate in internal medicine, general surgery wards and outpatient and emergency medical personnel (p lt; 0.05). Wash hands after all indications appear, hand hygiene compliance rates in order to deal with the higher dirt after going to the bathroom before contact with blood, body fluids, secretions, excretions, wound dressings, the were 74.71 ± 38.99% , 63.85 ± 41.93%, 51.214 ± 43.99%. Affecting the medical staff perform hand hygiene 20 factors, the most important top three factors are: ① detergents, disinfectants irritate the skin, causing skin dry; ② busy work is not enough time: ③ individuals at work right hand or towel without sufficient attention. (2) medical staff hand hygiene knowledge level: 180 medical staff hand hygiene knowledge average score 21.37 ± 1.50 points, including hand hygiene indications score 16.11 ± 1.12 points, hand hygiene knowledge was 5.27 ± 0.75 points. Different age, gender, occupation, title, education, seniority medical staff hand hygiene knowledge scores were no significant differences (p gt; 0.05), critical care ward medical staff hand hygiene knowledge scores than medical, surgical ward and emergency door Nurse (P lt; 0.05). Medical staff hand hygiene knowledge scores and hand hygiene compliance rate by the statistical test no correlation (p gt; 0.05). 3 hands against medical personnel status and cognitive behavioral interventions main factors for control experiments: the experimental group behavior education Wash hands after passing rate of 58.33% before intervention increased to 91.67%, 27.55% hand-washing rate rose to 41.60% the control group, a pass rate of 66.67% hand-washing, hand-washing rate of 28.68%, self-control and post-intervention compared with the control group, there were significant differences between the two (P lt; 0.05); experimental group provides quick hand disinfectants, Rub your hands with disinfectant hand sanitizer compare the effect flowing water wash, wash the passing rate was not statistically significant (P gt; 0.05); bacterial eradication rate using hand sanitizer as 88.83 ± 2.50%, hand sanitizer flowing water was 86.67 ± 3.22%, there is a significant difference between the two (p lt; 0.05); perform hand clinic with a time using the hand sanitizer as 19.33 ± 4.33s, hand sanitizer flowing water was 77.67 ± 30.92s, there are two significant differences (P lt ; 0.01); patients on the ward patients and their families including health promotion, including hand after hand washing rate rose to 70.97 ± 12.07%; experimental group before the intervention, behavioral education, they provide quick hand sanitizer, hand of patient participation Wash hands after health promotion rate comparison showed significant differences (P lt; 0.05); experimental period in the intervention group compared with the control group hospital infection rates were significantly different (P lt; 0.05). Conclusion The medical staff hand hygiene situation must be high to perform hand hygiene and hospital infection control awareness needs to be strengthened; through the adoption of targeted interventions can effectively improve the medical staff hand hygiene conditions and perform hand hygiene compliance, reduce hospital infections rate.
