Combined Assays of Tumor Marker Vimentin, ER, CEA, CK8/18 and CK8/18-mRNA for Differential Diagnosis of Primary Endometrial and Cervical Adenocarcinoma

Abstract


Objective:To study the clinical significance of tumor markerscombined assay in the differential diagnosis of primary endometrial andcervical adenocarcinomas. Methods:A tissue microarray was constructed using paraffin-embeded,formalin-fixed tissues from 61 hysterectomy specimens.Among them, 35cases were endometrial adenocarcinomas,26 cases were cervicaladenocarcinomas.Tissue array sections were immunostained with fourcommercially available antibodies (vimentin,ER, CK8/18, CEA) utilizingthe avidin-biotin(ABC) tichnique.The other one section was stained withCK8/18-mRNA utilizing the in situ hybridization tichnique.Positive ratewas count respectively. Results: Positive rates of vimentin, ER, CK8/18, CK8/18-mRNA,CEA in endometrial adenocarcinomas were 74.3%,54.3%, 91.4%,65.7%, 40.0% respectively;the positive rates in cervical adenocarcinomaswere 11.5%,11.5%,50.0%,15.4%,92.3% respectively.The positive rates ofvimentin, ER, CK8/18 and CK8/18-mRNA in endometrialadenocarcinomas were higher significantly than those in cervicaladenocarcinomas(P<0.005), the positive rate of CEA in endometrialadenocarcinomas was lower significantly than that in cervicaladenocarcinomas(P<0.005). When combined together,the diagnosticconsistency of combined assays with vimentin+ER+CK8/18 +CEA was thesame as the combined assays with vimentin+CK8/18+ CEA(the total was93.4%, endometrial adenocarcinomas was 91.4%, cervicaladenocarcinomas was 96.2%).Both were higher than that of combinedassays with vimentin+ER+CEA(the total was 86.9%, endometrialadenocarcinomas was 85.7%, cervical adenocarcinomas was 88.5%).Thediagnostic consistency of combined assay including CK8/18-mRNA werehigher than the other assays. Conclusion:Tumor marker of CK8/18 has significantly clinicalvalue in the differential diagnosis of primary endometrial and cervicaladenocarcinomas.When combined with others, The diagnostic consistencyof combined assay including CK8/18-mRNA were higher than CK8/18.
