Objective: To explore skull base meningiomas Chuo surgical treatment strategies and principles . Meningiomas microsurgical treatment of 74 cases of skull base Chuo Methods: From January 2002 to April 2005 were treated 74 cases of patients with skull base central meningiomas , which originated in the Medial sphenoid ridge and anterior clinoid 35 cases originated in the saddle compartment , saddle-node section 19 cases originated in the parasellar , nine cases of the cavernous sinus , petroclival meningiomas 11 cases . All patients underwent microsurgical technique under general anesthesia to remove the tumor . This paper reviews the analysis of the clinical data of the above cases , in order to explore central for different types of skull base meningiomas , how appropriate surgical approach , microsurgical technique , effective treatment . Results: 59 cases of total resection of tumor , tumor full cut the percentage : the saddle nodules and the saddle compartment meningioma 89% ( 17 / 19 ) , anterior clinoid and medial sphenoid ridge meningioma accounted for 86% ( 30 / 35) , petroclival meningiomas accounted for 64% ( 7 / 11 ), cavernous sinus meningiomas accounted for 55% ( 5 / 9 ) , subtotal resection in 15 cases , 2 cases of oculomotor nerve palsy , 1 case of postoperative brain within the hematoma , hematoma clear uneventful recovery , one case of postoperative cerebrospinal fluid leakage , intracranial infection cases and 2 deaths ( respectively, died of acute renal failure and lung infection ) . Serious early postoperative complication rate was 11% . Follow - up of 6 months to 2 years , subtotal resection in 5 patients relapsed . Conclusion : the central skull base meningiomas should actively take surgery to take the appropriate surgical approach by preoperative imaging , intraoperative application of microsurgical techniques , fine operation , most skull base central meningiomas get the success surgical excision.
