Abstract


Objective: The aim of this study is to determine the limit to tolerable in situ warm ischemia time for lung grafts after circulatory arrest,and to investigate the trigger of ischemia-reperfusion injury in lung transplantation from non-heart-beating donor.Methods:Thirty New Zealand white rabbits were randomly divided into three groups : heart-beating donor (HBD), non-heart-beating donor with 30 minutes of warm ischemia time(NHBD-30) and non-heart-beating donor with 60 minutes of warm ischemia time(NHBD-60). Each group has 5 pairs (the donors and the recipients). Tracheostomy tube was performed, and mechanical ventilation was established after the donor was anesthetized . All animals received heparin,then the lungs belonging to HBD group were retrieved immediately after death, but the animals belonging to NHBD-30or NHBD-60 group were left at room temperature for 30 or 60 minutes after circulation ceased, and ventilation was maintained until harvest time. All lungs were flushed with modified LPD solution, and the heart-lung block was harvested. The entire block was immersed in modified LPD solution and stored at 4°C. Rabbit left lung transplant model were used. The right pulmonary hilum were intercepted 4 hours after reperfusion, then the data were observed.Results:1. All the recipients of heart-beating lungs and 30 minutes cadaver lungs survived the observation period with excellent gas exchange, whereas 2 of 5 recipients of 60 minutes cadaver lungs survived for 15 minutes after the ligation of right pulmonary hilum and 2 for 20 minutes, and the data show significant difference compare with
