Background since this century, the incidence of diabetes around the world is increasing at a speed of nearly 3% per year. As one of the major chronic complications of diabetic nephropathy (diabetic nephropathy DN) Each year, about 30% -40% of patients progress to end-stage renal disease, is one of the main causes of death of patients with diabetes. Therefore, the prevention and treatment of diabetic nephropathy, development is a topic of great concern. Its pathogenesis has not been fully elucidated, now is the result of the combined effects of the two factors of renal hemodynamic and non-hemodynamic. In recent years, the vasoactive peptide renal hemodynamics concern. Ever found a group of vasoactive peptide endothelin (ET) and calcitonin gene-related peptide (CGRP), the former is the strength of the duration of the longest vasoconstrictor peptide, CGRP has the strongest non-adrenergic, non- cholinergic dependent vasodilatory effects. The balance between the two to maintain the renal hemodynamic stability. This article by detecting urinary albumin excretion rate in patients with type 2 diabetes grouping and control each group was analyzed in plasma ET and CGRP level to explore the relationship with the development of diabetic nephropathy, which for further research on the pathogenesis of diabetic nephropathy, as prevention and control work to provide some ideas. Objective To investigate plasma calcitonin gene-related peptide (CGRP) and endothelin (ET) in patients with type 2 diabetic nephropathy trend, explore the relationship between CGRP, ET, and the pathogenesis of diabetes. The method 25 normal healthy controls and 83 patients with type 2 diabetes patients included in this study. Diabetic patients were divided into normal albuminuria group (28 cases) according to urinary albumin excretion rate (UAER), microalbuminuria group (29 cases) and clinical albuminuria group (26 cases). Plasma CGRP and ET content detected by radioimmunoassay, while blood glucose, blood lipids, blood renal level. Type 2 diabetes and diabetic nephropathy in patients with plasma ET values ??compared to the normal control group significantly increased (P <0.01) and with UAER was being related (r = 0.65, P <0.01); diabetic nephropathy patients CGRP compared with the control group was significantly decreased (P <0.05 or <0.01), and was negatively correlated with UAER (r = -0.71, P <0.01), ET and CGRP showed a significant negative correlation (r = -0.62, P <0.01). Conclusion CGRP and ET imbalance in the level of participation in the incidence of diabetic nephropathy, development, resulting in a deterioration of renal function. In the early stage of diabetes, maintain a dynamic balance of CGRP and ET conducive to the prevention and treatment of diabetic nephropathy and development.
