Objective gastrointestinal stromal tumor (GIST) is derived from the gastrointestinal mesenchymal tumors, including previously known as leiomyoma, leiomyosarcoma most mesenchymal tumors. Stomach and intestinal stromal tumor gastrointestinal stromal tumors accounted for 80%. Currently immunohistochemistry CD 117 and CD 34 is a gastrointestinal stromal tumor diagnosis necessary conditions. However, its clinical different GIST tumor biology are very different. Therefore, there is a tendency: that are potentially malignant GIST, promote the use of risk classification to replace the previous simple benign and malignant classification. The present study was the degree of malignancy grading standards GIST comments yet unified, it also poses a challenge to us, how to use one or several detection metrics to accurately determine the extent and prognosis of malignant GIST. Tumor cells with the gene mutation or damage, whereas the type of malignant tumor cell proliferation and tumor activity of closely related. Therefore, from cancer development and the root causes of GIST up study biological characteristics of malignant transformation of the law, but also help us to rediscover gastrointestinal stromal tumors. In this study, based on the above considerations, by detecting the reflection of the kinetics of tumor proliferation index of proliferating cell nuclear antigen (PCNA), as well as with proliferation, transformation and tumorigenic potential of mutant p53 gene in different malignant GIST, the positive expression levels to explore Gastrointestinal stromal tumors p53 protein and PCNA expression and malignant transformation occurs with the relationship. Meanwhile, the two may also become a clinical judgment of gastrointestinal stromal tumor malignancy important reference index. Again, the latest developments in the treatment of GIST is inclined to molecular biology targeted therapy (imatinib application) and surgical treatment of the combination, the combination of the direction of change. p53, PCNA in different malignant GIST whether the level of expression can also timing of medication after surgery to provide clinical guidance? This is what we expect. Experimental methods a. Cases All cases were graded according to the light microscope morphological characteristics, combined with immunohistochemical staining results (CD 117 -positive and / or CD 34 -positive) to make the diagnosis of GIST and re-grading . Two. Specimen samples were 4% formalin-fixed, paraffin-embedded serial sections (4μm), HE staining and immunohistochemical staining. Immunohistochemical staining by microwave antigen retrieval, according to SP method, DAB color. III. Immunohistochemical staining results to determine with known breast cancer tissue sections as positive controls. With PBS instead of primary antibody as a negative control. 128 cases of GIST determination of p53 protein and PCNA expression levels. Four. Statistical analysis of data used to represent the rate difference between the groups using x 2 test. All the data is processed using SPSS11.5 software. Results 1.128 cases of GIST, p53 expression levels in low-grade malignant group was 7.1%, in the low-grade group was 45.0%, in the intermediate grade group was 72.6% in the highly malignant group was 93.8%, in ascending order that expression levels vary the degree of malignancy rises gradually increased, four pairwise comparisons, the differences were statistically significant (P <0.05). 2.128 cases of GIST, PCNA expression levels in low-grade malignant group was 0% in the low-grade group was 40.0%, in the intermediate grade group was 69.4%, in the highly malignant group was 90.6%, which in ascending order that expression levels were also increased with the degree of malignancy GIST gradually increased, pairwise comparisons between groups, differences were significant (P <0.05). Conclusions 1. p53, PCNA expression levels are closely related with the nature of GIST, once again demonstrated tumor cell proliferation and apoptosis of GIST imbalance is one of the mechanisms of malignant transformation. 2. p53, PCNA can be judged clinically malignant gastrointestinal stromal tumor extent an important reference index. 3. p53, PCNA in different malignant GIST, the positive expression levels may be the timing of medication after surgery to provide clinical guidance.
