Background: Since the spine bone graft after posterolateral fusion is a common clinical spinal fusion, but the method is still some unresolved issues, such as the potential for bone area of ??infection and intractable pain, surgery time and intraoperative blood loss increase. It is estimated that a single interbody since about 5% -35% of the iliac bone graft spinal fusion patients not healing. Even if supplemented by internal fixation is still 10% -15% of patients with spinal fusion pseudarthrosis occurred. In order to solve these problems, a number of bone graft substitutes came into being, the most widely used is the bone morphogenetic protein. It is an osteoinductive ability cytokines, but after years of research and clinical applications, some scholars have found some imperfections in BMP as heterotopic ossification ability too, to stimulate the transplanted area bone overgrowth caused some complications. BMP physiological dose increased health care costs. Therefore, to further explore better bone graft substitutes has become a necessity. Objective: To evaluate the demineralized bone matrix (DBM) as a carrier by adenovirus-mediated Nell-1 gene in rat spinal posterolateral fusion model of osteogenesis. Nell-1 is a new type of the osteoinductive protein found in craniosynostosis patients with early closure. After analysis Nell-1 can lead to the maturation of the bone and accelerate bone formation. Methods: 20 Athymic rats were divided into two groups, one implant adenovirus-mediated Nell-1 gene, another group implanted adenovirus-mediated LacZ gene, are DBM as the carrier. All rats spinal posterolateral fusion was killed six weeks after surgery. X-ray radiography rat 2,4,6 weeks, six weeks after hand touch mechanics assessment, the MicroCT inspection and histological sections observed. The results: six weeks after adenovirus-mediated AdNell-1 genome fusion rate was significantly higher than the AdLacZ group. Hand touch mechanics assessment and MicroCT check, AdNell-1 group fusion rate was 80% and 20% fusion rate of AdLacZ. Groups; histological sections observed, AdNell-1 group fusion rate was 70% fusion rate of AdLacZ. Groups 20%. Observation of bone maturity AdNell-1 group has a higher bone maturity. Conclusion: DBM carrier by adenovirus-mediated Nell-1 gene can be formed in the rat spinal posterolateral good bone fusion. Because it can be more loyal than BMP role in proliferation of bone cell differentiation, Nell-1 gene will become a promising osteoinductive factor. This experiment also proved adenovirus as a gene therapy vector system is very stable and effective, while MicoCT as a new evaluation of the spinal fusion method can be more accurate guide to the high-quality image, and according to the three-dimensional reconstruction, to accurately evaluate the bone structure of the entire spine, worthy of promotion.
