Abstract

Objective: metabotropic glutamate receptor subtype 5 (mGluR5) is a set of novel G protein-coupled receptors exist in the mammalian brain and spinal cord, excitatory nerve center involved in inflammatory and neuropathic pain The transfer process, the activation of mGluR5 receptors will enhance the state of pain. mGluR5 receptor selective antagonist (MPEP) analgesic effect. The pulp distribution of intensive sensory nerve stimulation is delivered by pain signals pain, trigeminal nerve from the brain. In recent years, experimental studies have shown that in the normal pulp coronal pulp odontoblasts distribution of mGluR5, mGluR5 expression and distribution of inflammation of the pulp or other parts of the normal pulp is not clear in this experiment by immunohistochemistry method to detect the normal pulp various parts and inflammatory pulp coronal pulp mGluR5 expression and distribution; using image analysis system, a semi-quantitative analysis of its expression intensity of the mGluR5 in the normal pulp and inflammation pulp The role and significance of a new idea for targeted therapy clinical pulpitis. Methods: 1. Collect oral and maxillofacial surgery patients Shandong University Dental Hospital for orthodontic or other treatment needs extracted human premolars or third molars five cases, as pulp Health group; clinical and pathological diagnosis of acute pulpitis and five cases of chronic pulpitis each specimen consecutive tissue sections, HE staining and immunohistochemical staining of phosphate buffer (phosphatic buffer solution, PBS) instead of primary antibody as a negative control. HE staining: the normal pulp Crown marrow were high columnar odontoblasts were cuboidal in the neck and the central root, and was flat in the apical area; chronic pulpitis odontoblast irregular arrangement, the continuity of the cell layer is damaged, the pulp tissue in a large number of chronic inflammatory cells, mainly lymphocytes and plasma cells; the acute pulpitis under the carious lesion odontoblasts continuity is destruction, pulp vascular dilatation and congestion , increased vascular permeability, fluid leakage, tissue edema, neutrophil infiltration around along the vessel wall. 3 immunohistochemical results: the normal pulp the crown, neck to the crown of the root pulp and inflammation of the pulp marrow odontoblast expression of mGluR5 positive for its distribution: normal pulp by the crown, neck Ministry to the root of the the mGluR5 expression of strength in order of decreasing; acute pulpitis chronic pulpitis and normal pulp coronal pulp mGluR5 expression were decreased. 4.mGluR5 immunohistochemical results, the average optical density detection: by alpha = significance level of 0.05, the normal pulp the crown, neck and root mGluR5 mean optical density value was statistically significant (P lt; 0.05), LSD method group pairwise comparison between both P less than 0.001, indicating that the normal pulp to the root of mGluR5 expression were decreased by the crown, neck; acute pulpitis chronic pulpitis and normal pulp coronal pulp mGluR5 mean optical density statistically significant (P lt; 0.05), LSD method group pairwise comparison between P value less than 0.001, indicating that the acute pulpitis chronic pulpitis and the normal pulp of the coronal pulp mGluR5 expression levels were decreased. Conclusion: 1. Normal pulp and inflammatory pulp odontoblasts expression of mG1uR5. 2 normal pulp to the root of the the mGluR5 expression of strength by the crown, neck turns; acute pulpitis chronic pulpitis and normal pulp coronal pulp mGluR5 expression were decreased. Tip of the mGluR5 has a certain role in the transfer process pulp pain. Inferred application the mGluR5 receptor selective antagonist (MPEP) can block the delivery of endodontic pain, provides a new way of thinking for the targeted treatment of clinical pulpitis.
