Abstract


Research background: T2DM is a chronic metabolism disease in short of insulinoppositely or absolutely which is induced by several reasons. Insulin resistance plays animportant role in T2DM. Strengthening the insulin sensitivity is the basic of the treatment.We made the clinical research on Simiao Granules treating T2DM IR of syndromedampness-heat puzzling spleen by the guidance of TCM.Research purpose: To observe the functions of Simiao Granules improving IR anddiscover the mechanism, make the new theory foundation of TCM treatment.Research content: The thesis is consisted of theory study and clinical study. Theorystudy divided into two parts. The first part expounds the related records of XiaoKe, thepathogenesis study of IR and the progress of experimental and clinical study in TCM. Thesecond part illustrates the definition, popularity and mechanism of IR, expounds therelative diseases and the treatments.In Clinical study, we selected 43 T2DM IR patients ofsyndrome dampness-heat puzzling spleen. The treat group (simiao granules) contains 22cases and the control group(Metformin) contains 21 cases. We observed the BMI, FPG,2hPG, Insulin, blood fat, insulin sensitive index, CRP, TNF and syndrome scores beforeand after treatment.. The research lasted 8 weeks.Research results: Many indexes have been improved after treatment both in controlgroup and treat group, and they had statistical significance after analysis. 1, glucosemetabolism, compared FPG and 2hPG of two groups before and after treatment, the datahad statistical significance,but the change of HBA1C is not obviously(P>0.05).Comparing the corresponding index after treatment of two groups, it’s no statisticalsignificance(P<0.05); 2, lipid metabolism, TG and LDL of treat group decreased aftertreatment(P<0.05). TG of control group decreased also; the change of LDL,TC HDL intwo groups has no statistical significance(P<0.05).3, The change of CRP and TNF in twogroups has obvious statistical significance(p<0.05), but it’s no statistical significancecomparing the decreased level of two groups(p>0.05).4, syndrome scores, the improvement of treat group is better than control group.5, BMI and IAI, it’s no statisticalsignificance of two groups comparing BMI before and after treatment; but the comparisonof IAI has statistical significance(p<0.05),and the comparison of IAI in two groups is nostatistical significance(p>0.05).Conclusion: Simiao granules improves the symptoms of the T2DM IR patients ofsyndrome dampness-heat puzzling spleen, and it could improve the IR of the patients byseveral methods such as controlling the blood glucose, adjusting blood fat and decreasingthe level of irnflammation factors.
