Meta Analysis of the Relationship between IL-6 or TNF-α and Human Primary Hepatocellular Carcinomas

Objective:Accumulating data from basic research suggest that interleukin-6 and tumor necrosis factor-αplay critical roles in the promotion and progression of hepatocellular carcinomas. However, results from investigation of levels of interleukin-6 and tumor necrosis factor-αin the blood of patients with hepatocellular carcinomas are controversial. We use meta-analysis to evaluate the relationship between interleukin-6 or tumor necrosis factor-αand human hepatocelluar carcinomas.Methods:Meta-analysis is used determine the levels of interleukin-6 and tumor necrosis factor-αin the serum or plasma from patients with hepatocellular carcinomas by reviewing all the relevant case control studies in libraries such as Pubmed、ISI web of knowledge、CBM and CNKI. Two authors collected data independently and evaluated quality and results.Results:(1) Meta-analysis of the relationship between interleukin-6 and hepatocellular carcinomas:Twenty-five articles about Interleukin-6 were included in the meta analysis. The results showed that the level of Interleukin-6 in the blood from the patients with hepatocellular carcinomas was higher than the health(25 articles SMD=3.74 95%CI 2.98~4.51), patients with cirrhosis of liver (13 articles, SMD= 1.43,95%CI 0.78~2.08) and patients with hpatitis (6 articles, SMD=1.17,95%CI 0.29~2.05)。(2) Meta-analysis of the relationship between tumor necrosis factor-αand hepatocellular carcinomas:twenty-three articles about tumor necrosis factor-αwere included in the meta analysis. The results showed that the level of tumor necrosis factor-αin the blood from the patients with hepatocellular carcinomas was higher than the health (23 articles SMD=2.55 95%CI 1.93-3.16), but there was no significant difference between hepatocellular carcinoma and hepatitis (5 articles, SMD= 0.40,95%CI-0.2-0.99). There was also no significant difference between hepatocellular carcinoma and cirrhosis of liver (14 articles, SMD=0.49,95%CI-0.26~1.26)Conclutions The results indicated that interleukin-6 and tumor necrosis factor-αin hepatocellular carcinoma patients showed higher levels than the normal persons. These results suggest that interleukin-6 and tumor necrosis factor-αmay be involved in tumorigenesis of hepatocellular carcinomas. It is feasible to apply anti-inflammation treatment targeting interleukin-6 or tumor necrosis factor-αin therapy of human hepatocellualr carcinomas. Our study provide basis for better therapy of hepatocelluar carcinomas.
