Abstract


Objective:Colorectal cancer is one of the most common malignant tumors, for nearly30years incidence and mortality rates are on the rise, in our country’s malignant tumor of mortality in fourth place. The treatment preferred removed surgically, the5-year survival rate of about50%. Related research shows that, surgery combination chemotherapy can reduce tumor recurrence and metastasis and improve survival, for not the operation or advanced cancer patients, chemotherapy appears more important. Along with the progress of molecular biology and tumor internal medicine, chemical treatment has become an effective treatment and important means of colorectal cancer, the drug resistance is one of the main reasons which seriously influences the chemotherapy effect and survival of colorectal cancer patients. Among them, multidrug resistance (MDR) mostly has clinical significance. Multidrug resistance is that after tumor cells develop resistance to certain chemotherapy drugs, which also produce a similar resistance for mechanism and chemical structure of different chemotherapy drugs. Related research shows that multidrug resistance is more than a stage of the development of integrated complex events involving many factors. Research proves that, in tumor cells, COX-2can regulate the expression of MDR1/P-gp, which notes COX-2and multidrug resistance are related. But related research reports in which we certainly guide gastrointestinal cancer chemotherapy by COX-2expressions are still less. Topo is a basic ribozyme which can catalytic DNA local super coiling conformation changes, it is divided into Ⅰ、Ⅱ class, among them, Topo-Ⅱ is closely related cell with resistance. Resistance mechanism of Topo-Ⅱ mainly displays in Topo activity, expression reduce or gene mutation, so that the targets of anti-cancer drugs reduce or loss, produce resistance. Topo-Ⅱ is a kind of indispensable important nuclear enzymes, which participate in transcription, translation, replication, chromosome segregation and other processes of DNA of eukaryotic cell, which can promote tumor cell DNA synthesis and improve the proliferative capacity of tumor. Therefore, the study detect the expression of Topo-Ⅱ and MDR related factor colorectal cancer cells using immunohistochemical method, detect the correlation of them with age, gender, tumor size, anatomical site, histological grade, clinical stage and lymph node metastasis, preliminarily study on their relationship and clinical significance.Methods:We select surgical resection specimen of colorectal cancer patients in our hospital from January2005to March2010, all specimens are fixed with10%formaldehyde solution, paraffin-embedded sections and diagnosed by HE staining organization, all cases of colorectal cancer are no radiation and chemotherapy before surgery, biopsy specimens are operated with dewaxing, repair, incubation, washing, staining, dehydration, transparent, specimens of sealing, microscope imaging and image acquisition and analysis by immunohistochemical SP method, carries on the result determination about expression in sections, statistical analysis using SPSS15.0statistics software package.Results:The positive expression rate of COX-2in colorectal carcinoma is significantly higher than the positive expression rate in the paraneoplastic and normal tissues, P<0.01; the expression of COX-2in colorectal carcinoma has nothing to do with the patient’s age, tumor size, invasion extent, degree of differentiation, pathological type, while lymph node metastasis and dukes staging are closely related, P<0.05. The expression of Topo-II in colorectal cancer, their cancer-adjacent tissues and normal tissues were not statistically significant, P>0.05; the expression of Topo-Ⅱ in colorectal cancer might have nothing to do with the clinicopathological factors such as the patient’s age, tumor size, extent of invasion, degree of differentiation, pathological type, lymph node metastasis and ducks installment, P>0.05. In colorectal cancer, the expression of COX-2was positively correlated with that of Tope-Ⅱ, and it was statistically significant(r=0.254,P<0.05), while the expression of COX-2and Tope-Ⅱ in adjacent tissues of colorectal cancer and normal tissues was not significantly associated, P>0.05.Conclusion:1.The expression of COX-2in colorectal cancer increases and significantly be higher than the adjacent tissues, that explain excessive expression of COX-2may be involved in the formation process of colorectal cancer, the expression of Topo-Ⅱ in colorectal cancer, their cancer-adjacent tissues and normal tissues have no significant differences.2. The expression of COX-2in colorectal cancer has nothing to do with the patient’s age, tumor size, invasion extent, degree of differentiation, pathological type, while lymph node metastasis and dukes staging are closely related, it shows that COX-2plays an important role in the process of metastasis of colorectal carcinoma, the detection of COX-2expression provides the basis for early diagnosis of colon cancer and whether the occurrence of tumor metastasis; The expression of Tope-Ⅱ in colorectal cancer might have nothing to do with the clinicopathological factors such as the patient’s age, tumor size, extent of invasion, degree of differentiation, pathological type, lymph node metastasis and ducks installment, the expression of Tope-Ⅱ in colorectal cancer should not be used as indicators to judge by the degree of malignancy of colorectal cancer, which can be used as independent prognostic factors that determine the response to chemotherapy.3. The expression of COX-2was positively correlated with that of Tope-Ⅱ, and were statistically significant, it shows that COX-2and Tope-Ⅱ may have certain common function in tumor MDR mechanism, the combined use may has a synergistic inhibitory effect on colorectal cancer metastasis and resistance, helps early diagnosis of colorectal cancer and potential judgment of lymph node metastasis.
