The First Part The Expression of HIF-la, SOCS-3 and Bax in Myocardium in the Cases of Sudden Cardiac DeathObject To detect the expression of HIF-1a, SOCS-3 and Bax in myocardium in patients of sudden cardiac death and the relationship of HIF-la, SOCS-3, Bax and sudden cardiac death, to investigate their correlations of HIF-1a and Bax, and provide an objective basis for forensic identification of SCD.Method Twenty-five cases of SCD were chosen as the experimental group while twenty-five cases of non sudden death as the control group. Immunohistochemical staining was employed to examine the expression of HIF-1α、SOCS-3 and Bax in myocardium.Results1. HE stainingIn the SCD group, myocardial fiber fracture, myocardial interstitial edema, interstitial hemorrhage and a small amount of lymphocytic infiltration were observed in 20 cases of sudden coronary death, myocardial interstitial fibrous connective tissue was increased, myocardium was hypertrophy and myocardium was fatty infiltration, eosinophilia was enhanced, part of myocardial cell was degenerative and necrotic, myocardial cells were slightly edema, myocardial nucleus was slightly increased, and abnormal nucleus was observed, chromosome was loose. Left anterior descending coronary artery, circumflex branch and right coronary artery were noticed thickening in intimal and the degree of luminal stenosis was>50%, part of the lumen occluded, part of lumen was thrombosed.2. Immunohistochemical staining(1) The positive expression of HIF-la was detected in the cytoplasm in each case of SCD group; the positive expression was detected in the cytoplasm in 3 cases of the negative control group; the expression of HIF-la in the group of SCD was significantly higher than in the negative control group (Uc=6.083, P<0.01);(2) The positive expression of SOCS-3 was detected in the cytoplasm in 22 cases of SCD group; the positive expression was detected in the cytoplasm in 2 patients of the negative control group; the expression of SOCS-3in the group of SCD was significantly higher than in the negative control group (Uc= 5.8306, P<0.01);(3) The positive expression of Bax was detected in the cytoplasm in each case of SCD group; the positive expression was detected in the cytoplasm in 3 cases of the negative control group; the expression of Bax in the group of SCD was significantly higher than in the negative control group (Uc =5.573, P<0.01);(4) The expression of HIF-1αand Bax in myocardium of patients with SCD was positively correlated (rs=0.736, P<0.01).Conclusion1. HIF-1a was a reflection of myocardial ischemia, a very sensitive indicator of ischemia, the expression of HIF-la in myocardium in patients in acute myocardium ischemia with sudden cardiac death was early significantly increased. HIF-la was a sensitive marker for early acute myocardial ischemia and could be used to help the diagnosis of sudden death due to acute myocardial ischemia. It could also be used to distinguish the sudden death causes between acute myocardial ischemia and others such as asphyxia.2. The expression of SOCS-3 in myocardium in patients in acute myocardium ischemia with sudden cardiac death was early significantly increased, it is suggested that SOCS-3 may be involved in the pathogenesis of SCD.It can be used for diagnosis of SCD early (1 to 6 hours), rapid and sensitive morphological auxiliary indicators.3. The expression of and Bax in the myocardium of SCD indicates the involvement of apoptosis which contributes to the myocardium ischemia. It is also the early signs of myocardium ischemia. Bax might become the sensitive, reliable markers for the forensic diagnosis of SCD.4. Increased expression of HIF-1αand Bax are correlated with the incidence of SCD to some extent, they may play synergistic action in the process of SCD pathological changes, and participate in regulation of myocardial cell apoptosis. HIF-1αmay regulate Bax, thus promote myocardial cell apoptosis.The Second Part Study on DNA Extraction from Formalin-Fixed and Paraffin-Embedded Autopsy TissueObject To investigate the influence of three DNA extraction methods for formalin-fixed paraffin-embedded autopsy tissue on DNA quality, and to look for a simple, less pollution, economic and practical method for DNA extraction from paraffin-embedded autopsy tissue.Method 10 cases of myocardial tissue of formalin-fixed paraffin-embedded tissue were selected and divided into three groups according to the method for DNA extraction. Three methods included xylene dewaxing and phenol-chloroform method, improved TES dewaxing with water-bath and phenol-chloroform method, DNA extraction kit method. The quality of obtained DNA was analyzed by electrophoresis and PCR amplification.Results The DNA extraction efficiency of improved TES dewaxing with water-bath and phenol-chloroform method was same as the efficiency of xylene dewaxing and phenol-chloroform method. The A260/A280 ratio of DNA obtained by using both improved TES dewaxing with water-bath and phenol-chloroform method and xylene dewaxing and phenol-chloroform method, were statistically different from DNA extraction kit method(P<0.01). The aim trips were as bright as the positive control when using the DNA extraction by improved TES dewaxing with water-bath and phenol-chloroform method as the template.Conclusion The results demonstrated that improved TES dewaxing with water-bath and phenol-chloroform method is an optimal method for extraction of DNA from FFPET.
