Abstract


Purpose:1.To detect the expression of MMP-7 in peritoneal lavage in patients with gastric cancer under lapariscopic-assisted gastrectomy by flow cytometry and to explore MMP-7 clinical significance.2.To explored the infulence of laparoscopic gastric surgery to exfoliated cancer cells through the changes of MMP-7 expression in peritoneal lavage before and after laparoscopic gastric cancer surgery 3.To provide a theoretical basis for the safety of laparoscopic gastric cancer surgery and promote the field of minimally invasive surgery in gastric cancer further development and clinical application.Methods:We collected the peritoneal lavage of 50 patients with gastric cancer before and after laparoscopic gastric cancer surgery and examinated the expression of MMP-7 by flow cytometry and peritoneal lavage cytology. We collected another 10 cases of benign peritoneal fluid as a negative control, another 10 cases of gastric cancer tissue samples as a positive control.Results:1.The positive rate for MMP-7 in peritoneal lavage of 50 cases of gastric cancer was 52%(26/50), higher than the PLC positive rate which is 24%(12/50), the difference was statistically significant (P<0.05). The flow cytometry and PLC test results of MMP-7 in peritoneal lavage of 10 cases of benign gastric lesions were negative, 10 cases of gastric primary tumors, positive expression of MMP-7 were positive, and the quantitative value is much higher than the expression of peritoneal lavage specimens. 2.MMP-7 positive rate of detection is closely related with tumor invasion depth, TNM stage, and lymph node metastasis.3.The positive rate of MMP-7 for gastric cancer patients before surgery was 52%(26/50), the positive rate after surgery was 67.5%(27/40), the differences between the two groups was statistically significant (P>0.05). The PLC test results before and after surgery were 24%(12/50) and 34%(17/50) (P>0.05). The PLC results before surgery were positive while the FCM results were positive too, The PLC results after surgery were positive while the FCM results were positive, too. The PLC results were positive while the FCM results were positive, too.Conclusion:l.The detection of MMP-7 in peritoneal lavage may be a way to predict the peritoneal metastasis of gastric cancer.2.The method of FCM for detecting of MMP-7 in peritoneal lavage could be used as an indicator of the tumor malignancy and prognosis. 3.Lapariscopic-assisted gastrectomy did not increase the shedding of exfoliated cancer cells, and the minimally invasive advantages of laparoscopic surgery was obvious compared with traditional surgery. Lapariscopic-assisted gastrectomy is worthy of promotion and application.
