BACKGROUND AND PURPOSE: T-cell lymphoma to conventional chemotherapy is poor, pathology is characterized by tumor microenvironment significant inflammatory response, inflammatory cell infiltration and the presence of a variety of inflammatory cytokines; these inflammatory cells and inflammatory cytokines tumor cells to chemotherapy and poor relevance is unclear. In this study, previously untreated patients before by detecting tumor microenvironment number of macrophages and other inflammatory cytokine expression, combined with CHOP-based chemotherapy in patients with post-term evaluation and follow-up of long-term survival results of statistical Method of learning the correlation between them. Simultaneously by detecting tumor cells cyclooxygenase -2 (cyclooxygenase-2, COX-2) expression of COX-2 expression and investigate the number of macrophages and inflammatory cytokines secreted by the correlation between the expression and the media chemotherapy in turn has the effect. Analysis of the clinical features of T-cell lymphoma and the number of macrophages and COX-2 expression in the relationship between the observed T-cell lymphoma and clinical characteristics and their prognosis. Discussion of these findings for T-cell lymphoma tissue microenvironment on the impact of chemotherapy to find new ways to improve the efficacy of chemotherapy has a certain value. Methods: The pathological diagnosis clear, tumor tissue specimens, initial treatment line CHOP chemotherapy, and have a clear follow-up data of T-cell lymphoma, 45 patients entered the study. Application CD68 monoclonal antibody immunohistochemical tissue specimens (immunohistochemistry, IHC) markers, which calculated the number of CD68-positive cells and tissue specimens with IHC detection of macrophages secrete inflammatory cytokines IL-6, IL-8 protein expression and tumor cell expression of COX-2 protein. SPSS13.0 statistical package for data with statistics. Results: 45 patients with T-cell lymphoma tumor tissue macrophages in normal group, the number increased over recent chemotherapy group (ORR 88.8% higher than 44.4%, P = 0.008; CR 33.3% higher than 5.6%, P = 0.028) , the difference was statistically significant. IL-6, IL-8 expression were associated with chemotherapy efficacy and long-term survival has nothing to do. Negative expression of COX-2 positive expression group more effective than chemotherapy group (ORR94.1% higher than 57.1%, P = 0.010; CR35.3% higher than 14.3%, P = 0.100), COX-2 expression in macrophages There was a positive correlation between the number (r = 0.356 P = 0.017), but not IL-6, IL-8 expression was no significant correlation between. In addition to the number of macrophages in patients with short-term efficacy, PS score, prognostic index score, with or without bone marrow involvement related to external (P lt; 0.05), with age, sex, B symptoms, number of extranodal organs, LDH level, Ann Arbor installments were not correlated; while the COX-2 only with the Ann Arbor staging and related bone marrow involvement (P lt; 0.05), with other clinical features are irrelevant. Ann Arbor stage, LDH level, PS score, prognostic index score, with or without bone marrow involvement, number of extranodal organs, the number of macrophages and COX-2 expression levels and long-term survival (P lt; 0.05); gender , age, B symptoms, short-term effect, IL-6, IL-8 and the long-term survival are unrelated. Conclusion: The number of macrophages increased group and high expression of COX-2 and T-NHL chemotherapy and recent long-term survival was negatively correlated. IL-6 and IL-8 expression and chemotherapy response and long-term survival are unrelated. COX-2 expression and the number of macrophages was positively correlated but not IL-6, IL-8 expression was no significant correlation between. Clinical stage, LDH levels and bone marrow involvement was an independent factor affecting survival.
