Correlation between Total Ghrelin and Nutritional Status in Hemodialysis Patients

Abstract


Background:Malnutrition is a independent factor for predicting mortality and morbility in maintained hemodialysis(MHD) patients. Recent studies showed that Ghrelin leves are significantly increased, and they are correlated with nutritional status in hemodialysis patients.But this view remains controversy, the relationships between Ghrelin and nutritional status in hemodialysis patients are still unclear.Objectives:In this study,we measured plasma total ghrelin in MHD patients, and verity the correlation between ghrelin and other nutritional parameters.Methods:We included 30 MHD patients,18 chronic kidney disease(CKD)patients in 3～5 stage undergoing conservative treatment and 18 normal controls.The plasma total ghrelin levels were measured by enzyme linked immunosorbent assay (ELISA). We compared their plasma ghrelin levels with control group, the correlations between plasma ghrelin and nutritional parameters were also explored.Results:1. The plasma ghrelin levels in HD group were (4.55±2.34) ng/ml(pre-dialysis),and (2.27±1.12) ng/ml(post-dialysis) (P<0.0001); in CKD group and control group were (2.32±1.32) ng/ml and (1.99±0.83) ng/ml.The plasma ghrelin levels in HD group were significantly higher than that of CKD group and control group(P <0.0001). However,there was no difference between CKD group and control group (P=0.767)2. The plasma ghrelin levels in CKD3,CKD4,CKD5 groups were (1.73±0.81) ng/ml、(2.00±0.77) ng/ml及(3.72±1.83) ng/ml. The plasma ghrelin levels in CKD5 group were significantly higher than that of CKD3,CKD4 and control groups (P= 0.006). But there was no difference between CKD3,CKD4 and control group, and no difference between CKD5 and HD group (P=0.198)3. In HD group the plasma ghrelin levels were negatively correlated with pre-albumin (r=-0.461, P=0.010) and body mass index (r=-0.412, P=0.024)4. When all patients and controls combinded, the plasma ghrelin levels were positively correlated with serum creatinine (r=0.426,P<0.0001) and urea nitrogen (r=0.366,P=0.003); But negatively correlated with GFR (r=-0.411,P=0.001), pre-albumin (r=-0.321,P=0.009) and lymphocyte (r=-0.417,P<0.0001).Conclusions:1. The plasma total ghrelin levels in hemodialysis patients and CKD5 patients were much higher than that in healthy person and CKD3,CKD4 patients.2. Hemodialysis can significantly decrease the plasma ghrelin levels in hemodialysis patients.3. The plasma ghrelin levels were negatively correlated with pre-albumin and body mass index in hemodialysis patients.4. When all patients and controls combinded, the plasma ghrelin levels were positively correlated with serum creatinine and urea nitrogen, But negatively correlated with GFR, pre-albumin and lymphocyte.
