Objective To investigate the associativity between vascular mild cognitive impairment (VMCI) and homocysteine (Hcy) in patients with cerebral small vessel disease (SVD),to study the characteristic of early cognitive function impairment involvement of homocysteine in patients with cerebral small vessel disease.Methods A total of 151 patients with SVD were divided into VMCI group (80 patients) and non-VMCI group (71patients) according to MoCA scale.The levels of Hcy,folic acid and vitamin B12 were measured.Results 1. Plasma Hcy level of SVD patients in the VMCI group was significantly higher than that in non-VMCI group,plasma folacin and vitamin B12 levels in the VMCI group were significantly lower than those in non-VMCI group (P<0.01);2.The level of Hcy had positive correlated with age (r = 0.176,P <0.05), and negative correlated with total MoCA score (r=-0.482,P<0.01 )and subitem scores on spatial/executive ability,attention/counting,language,delayed recall (r=-0.280,-0.420, -0.227,-0.368,P<0.01).Conclusion 1. Hyperhomocysteinemia may be an independent risk factor for VMCI in SVD patients.Folacin and vitamin B12 deficiency is an important nutritional factor that may indirectly cause increase in Hcy and result in VMCI.2. The levels of Hcy in patients with SVD had negative correlated with the MoCA score,so the levels of Hcy possibly correlated with the degree of cognitive impairment;3. HHcy may participate in impairing the spatial/executive ability ,attention/counting,language and delayed recall ability and lead to cognitive impairment in SVD patients.
