Abstract


Objective To summarize the experience in diagnosis and treatment of intraductal papillary mucinous neoplasm(IPMN) of the pancreas and identify potential preoperative factors predicting invasiveness of intraductal papillary mucinous neoplasm of the pancreas.Methods Data from 27 patients of IPMN in PUMCH were retrospectively analyzed from 2003 to 2010.Results The average age of patients was 60.3 years old with a male to female ratio of 20:7. Abdominal pain, vomit, diarrhea, body weight loss and history of acute pancreatitis are common symptoms. Most of the tumors located in the head of pancreas. Pancreaticoduodenectomy and PPPD were mostly performed surgery methodologies. Pathological results revealed 15 cases of invasive IPMN and 12 noninvasive cases. Incidence of obstructive jaundice, tumor size and serum total bilirubin values were significantly different between the two groups. Other factors including sex ratio, age, incidence of abdominal pain or back pain, diarrhea, weight loss, new onset diabetes, serum CEA, CA19-9 values showed no statistical difference.Conclusions Preoperative diagnosis depends on radiological examination. Long-term follow-up of resected patients shows excellent outcome of noninvasive neoplasms, versus insignificant result of invasive neoplasms. Serum total bilirubin≥22.2μmol/L and tumor size could be predicting factors of invasive IPMN.
