Abstract


Objective:Oral Lichen Planus (OLP) is a chronic inflammatory disorder of oral mucosa. It is relatively common, often recurrent, itchy, not contagious. Oral lichen planus (OLP) occurs as the primary manifestation in 15-35% of patients of lichen planus. The most common sites affected are the buccal mucosa,tongue (mainly the dorsum), gingiva. According to statistics, the population prevalence of OLP is about 0.51%. Clinically, OLP presents as white striations, white plaques, erythema, erosions, or blisters. It includes the reticular, atrophic, hypertrophic, and erosive forms, or combinations of these subtypes. The mucosal lesions are extremely uncomfortable, painful and persistent. Histopathologically, it showed a dense subepithelial lympho-histiocytic infiltrate, increased numbers of intra-epithelial lymphocytes, and hydropic degeneration of basal keratinocytes. OLP is treated with anti-inflammatory agents, mainly the topical corticosteroids, but new agents and techniques are becoming available. Now this lesion OLP is recognized as a T cell-mediated autoimmune disease but The etiology of OLP remains unknown.In recent years we have reviewed studies of oral lichen planus, found that the current attention has focused on the hot strip was infiltration of lymphocytes and epithelial cells. However, fibroblasts are also involved in the repair process of inflammation, such as play an important role in the formation of granulation tissue. And fibroblasts (fibroblast, FB) in the role of the disease is poorly understood.The purpose of the present investigation was to detect and enumerate some cytokines producing cells in the fibroblast (FB) of patients with OLP, to compare them with those of healthy controls and to discuss the role that these cells may play in the pathogenesis of OLP. For this, we use enzyme-linked immunosorbent assay technique to detect the secreting and expressing of connective tissue growth factor(CTGF), Interleukin-6 (IL-6), transforming growth factor-β1(TGF-β1), matrix metalloproteinase -9(MMP-9) and (CXCL-10) in protein on connective tissue of vitro cultured normal human oral mucosa and oral lichen planus mucosa. Providing a theoretical basis for development and progression of OLP, at the same time provided new ideas for the clinical treatment of the disease in lichen planus.Methods:1. The study group consisted of 12 OLP patients and 9 healthy individuals OLP (n=12) and normal control (n=9) tissues were defined clinically and pathologically. Culture normal oral mucosa and lichen planus mucous connective tissue cells in vitro. Selected third to seventh-generation cells. Using ELISA technique to detect the secretion of connective tissue growth factor (CTGF), Interleukin-6 (IL-6), transforming growth factor-β1(TGF-β1), matrix metalloproteinase -9(MMP-9) and (CXCL-10) protein in each group. The results of every group are expressed with the average optical density (OD).2、Data of ELISA were analyzed by ANOVA and Student-t test, and expressed as the means±SD. Values of P<0.05 were considered statistically significant.Results:1. ELISA detection, CTGF and IL-6 secreted by the mucous of oral lichen planus is higher than secreted by the normal oral mucosa fibroblasts. Between the OD values of each group there were significant differences (P<0.05).2. However, no statistical difference was observed in the protein secreting of TGF-β1, MMP-9 and CXCL-10 in the fibroblasts of patients with OLP compared with the control group.Conclusion: Compared to normal oral mucosa, CTGF and Interleukin-6 showed a stronger expression in the tissue of OLP, and their distribution is different. The expression levels of Transforming growth factor-β1, Matrix metalloproteinase -9 and CXCL-10 protein secreting of OLP group are more than normal mucosa group, while, the expression level of the fibroblast shows no significant difference. Based on the role of fibroblasts previously mentioned, as well as the results of this experiment, provided a theoretical basis for the development and progression of OLP, at the same time provided new ideas for the clinical treatment of the disease.
