Abstract


Background and Purpose studies have shown that endometriosis with malignant potential. The malignant transformation of endometriosis mechanism is unclear. The purpose of this study is to investigate the PTEN and p53 gene mutations and abnormal protein expression of endometriosis in the malignant process. Methods Polymerase chain reaction and DNA sequencing, and immunohistochemical PV-9000 two-step detected 53 cases of organizations in the PTEN gene exon 2-8 mutation of the p53 gene 5-11 exon, as well as PTEN and p53 protein expression. Selected patients divided into three groups: 1, 23 cases of endometriosis, ovarian cancer (Endometriosis associated ovarian cancers, EAOCs) organizations, including the merger endometriosis ovarian endometrioid cancer nine cases, merge endometriosis ovarian clear cell carcinoma in 14 cases, 23 cases in four patients met the the Sampson-Scott endometriosis malignant diagnostic criteria; the 2 atypical endometriosis (Atypical endometriosis, aEMs) organization 10 cases; 3, of fresh ovarian endometriosis (Endometriosis 20 cases, EMT) organization. Results the 1.23 cases EAOCs, eight cases occurred PTEN gene mutation, the mutation rate was 34.8%, 4 cases of p53 gene mutation, the mutation rate of 17.4%, and 1 case of simultaneous PTEN and p53 mutations. Two cases of the endometriosis malignant standard cases, respectively, PTEN, p53 mutations. PTEN and p53 mutations in EAOCs group were significantly higher than aEMs group (0%) and EMT group (0%). Compared between EAOCs and aEMs group, PTEN, p53 mutation rate was no significant difference (P gt; 0.05), comparison between groups EAOCs and EMT, PTEN, p53 mutation rate differences were statistically significant (P lt; 0.01, P lt; 0.05). EAOCs in PTEN, p53 mutation rate with age, histological type, surgical - pathologic stage, histological grade and metastasis were unrelated (P gt; 0.05) PTEN in surgical - pathologic staging earlier, tissue differentiation. mutations in good cases is higher in surgical - pathologic staging late, organization poorly differentiated cases; p53 showed the opposite trend. On the same slice 2.EAOCs endometriosis tissue and cancer tissue components of PTEN protein expression. The p53 protein also showed the same trend. PTEN protein expression deletion rate of 78.26%, higher than the EMT group (50%) and aEMs group (20%), the p53 gene protein expression in the absence rate was 17.39%, lower than the EMT group (85%), and aEMs group (70%) . EAOCs of PTEN, p53 gene protein expression rate difference was highly statistically significant (P lt; 0.01), the difference was statistically significant (P lt; 0.05) compared with aEMs group compared with EMT. EAOCs in PTEN, p53 gene protein expression missing rate with surgery - pathological staging, pathological grading, is associated with metastasis-associated (P lt; 0.05), regardless of the age, histological type (P gt; 0.05), different is, PTEN surgical - pathologic stage, good tissue differentiation, metastasis-free EAOCs protein expression miss rate is significantly lower than the surgery - pathologic stage, poor tissue differentiation and (or) with metastasis, and p53 in surgical - pathologic stage, good tissue differentiation, metastasis-free EAOCs the lack of protein expression was significantly higher than surgical - pathologic stage, poor tissue differentiation and (or) with metastasis. PTEN and p53 protein expression was negatively correlated (r_s = -0.593, P = 0.003). Conclusion 1.PTEN and p53 gene mutation, the abnormal protein expression may be involved in the process of malignant transformation of endometriosis. The 2.EAOCs group PTEN and p53 mutations were significantly than aEMs group and EMT group showed that mutations in these two genes may be involved in the process of malignant transformation of endometriosis. PTEN mutations occur in cases of surgical - pathologic stage, histological grade, less malignant biological behavior and p53 gene mutations occur in surgery - late pathological stage, histological grade, degree of malignancy. Higher biological behavior of poor cases, suggesting that PTEN mutations may be an early event in the malignant process of the endometriosis, while p53 gene mutation may be a late event. The 3.EAOCs in PTEN protein expression in the absence of high the EMT group and aEMs group, p53 protein expression rate was lower than that EMT group and aEMs of group. The EAOCs in the same slice, endometriosis tissue and cancer tissue components, PTEN protein and p53 protein expression was not different, indicating that the abnormal expression of these two proteins may be involved in the process of malignant transformation of endometriosis.
