Abstract


Background diabetic nephropathy (DN) is the most common diabetes, one of the most serious complications. Some statistics indicate that in patients with end-stage renal failure in DN accounted for the first of about 36.39%, is expected to become a DN in chronic renal failure in developing countries the most important reason. So far, the DN modern medicine is no effective treatment methods to prevent its occurrence and deterioration. Chinese medicine in the prevention and treatment of diabetic nephropathy and development, delaying the progressive deterioration of renal function, has made great progress. TCM Clinical Treatment is the essence of TCM is to contact an important part of the theory and practice. Objective diabetic nephropathy syndromes overview, establish a unified TCM diagnosis, treatment standards, making differentiation more standardized. Method according to diabetes (DM) and diabetic nephropathy Western diagnostic criteria, select 200 cases of type 2 diabetes and diabetic nephropathy patients for the study, diabetic nephropathy meet diagnostic criteria for inclusion of patients in group A (120 cases, including retrospective analysis of 80 cases) . Meet Western diagnostic and duration of diabetes of 10 years and above excluded by examination of diabetic nephropathy patients were included in group B (80 patients, including retrospective analysis of 40 cases). Develop diabetes (diabetic nephropathy) symptoms graded quantization table and tongue and pulse quantization table for selected cases taken questionnaires completed questionnaire to fill; according to statistics required to complete the database creation, data entry, verification and management, and complete the relevant statistical analysis; consult industry experts on the main card Syndromes and advice, combined with the results of statistical analysis of diabetes (diabetic nephropathy) syndromes are classified, summarized. Results The study included a total of 200 cases of patients, 120 patients with diabetic nephropathy, including 15 cases of stage Ⅱ, Ⅲ of 28 cases, Ⅳ of 61 cases, Ⅴ of 16 cases. DN TCM clinical symptoms in patients with malaise and fatigue (69.2%), nocturia (60.0%), edema (56.7%) is more common, a little more than common diabetes symptoms, thirsty to drink (35.0%), easy to eat more hunger (31.7%), more frequent urination (32.5%), emaciation (15.0%), the performance is not obvious. According to internal organs deficiency diagnostic criteria, and found that the performance of DN patients with kidney deficiency syndrome dominated (37.5%), followed by spleen deficiency syndrome (19.2%), in order to permit the most rare lung deficiency (3.3%). Diabetic nephropathy Ⅱ, Ⅲ stage, Ⅳ period are highest incidence of kidney deficiency syndrome (each 53.3%, 39.3%, 37.7%), Ⅴ of spleen deficiency places the highest incidence (56.3%). According to the standards of yin and yang qi deficiency study found that patients with DN Yin deficiency still the main performance (32.5%), followed by qi deficiency syndrome (20.8%), deficiency syndrome (8.3%) and blood deficiency syndrome (10.0% ) is less common. Diabetic nephropathy Ⅱ, Ⅲ stage, Ⅳ of yin deficiency are the highest incidence (33.3% each, 32.1%, 32.8%). According to the standard composite deficiency study found that the composite deficiency in DN to Qi Deficiency (36.7%) is more common, followed by spleen qi deficiency syndrome (22.5%), both yin and yang deficiency (4.2%) and pulmonary kidney deficiency syndrome (5.8%) are less common, diabetic nephropathy, Ⅱ, Ⅲ stage, Ⅳ of Qi Deficiency are the highest incidence (each 60.0%, 28.6%, 37.7%), Ⅴ of places Spleen Qi deficiency highest incidence (37.5%). According to the standards of empirical studies found evidence of blood stasis DN patients (50.8%) is more common, followed by damp (27.5%), Qi stagnation (6.7%) are less common. Diabetic nephropathy Ⅱ, Ⅲ stage, Ⅳ stage, Ⅴ stasis of empirical occurrence rates were highest (each 46.7%, 57.1%, 49.2%, 50.0%). Conclusion diabetic nephropathy syndrome characterized mainly the following: deficiency in internal organs to kidney deficiency, spleen deficiency is more common, followed by a guilty conscience permits, liver and lung deficiency Deficiency; yin and yang to yin deficiency in certificate, Qi deficiency is more common, followed by blood deficiency, yang deficiency; composite in order to Qi Deficiency Deficiency, Spleen qi deficiency is more common, followed by liver and kidney, spleen and kidney yang imaginary, lung and kidney deficiency, yin and yang deficiency of both; Evidence to blood stasis, damp-heat syndrome is more common, followed by wet proof, heat proof, turbid damp, qi stagnation. Diabetic nephropathy is the vacuity of the card, virtual places Qiyinliangxu most common disease located primarily in the spleen; actually dominated by bleeding resistance, diabetic nephropathy late wet, wet cloud of evil than common. Qi, kidney collateral stasis is the key pathogenesis of diabetic nephropathy.
