Coronary heart disease and coronary artery disease patients with diabetes mellitus serum P- selectin and lipids, blood glucose correlation

Abstract


Objective: To observe coronary heart disease and coronary artery disease patients with diabetes mellitus P-selectin levels, analyze their blood glucose, blood lipid relationship between the evaluation of P-selectin, blood glucose, blood lipid platelet activation in coronary heart disease, and disease condition judgment, risk stratification and prognosis estimation significance. Methods: 30 cases of suspected cases as the control group, 102 cases enrolled patients, including 58 cases of coronary heart disease group (B), coronary heart disease group was divided into unstable angina group (B1 group) 28 cases and acute myocardial infarction group (B2 group) 30 cases; 44 cases of coronary heart disease and type 2 diabetes group (C), C group is also divided into unstable angina group (C1 group) 24 cases and acute myocardial infarction group (C2 group) 20 cases, respectively. Determination of serum P-selectin, fasting glucose, fructosamine, glycated hemoglobin, lipids and other indicators, and conduct general project inspection. P-selectin by enzyme-linked immunosorbent assay; gel filtration method using glycated hemoglobin, lipids series (including TC, TG, LDL, HDL) and fructosamine, fasting blood glucose were detected by conventional biochemical method. Results: 1) coronary heart disease and coronary artery disease in patients with diabetes mellitus serum P-selectin levels were significantly higher than the control group (P <0.05); 2) coronary heart disease and diabetes, coronary heart disease compared with the simple group, serum P - selectin levels were significantly higher (P <0.05); 3) CHD, coronary heart disease and diabetes group, acute myocardial infarction and unstable angina compared between the two groups, acute myocardial infarction, serum levels of P-selectin significantly increased (P <0.05); 4) in the case group P-selectin and serum low-density lipoprotein LDL was positively correlated (R ~ 2 = 0.498,0.572,0.468.0.517, P <0.05); positively correlated with TC ( R ~ 2 = 0.403,0.512,0.423,0.508, P <0.05); negatively correlated with HDL (R ~ 2 = -0.408, -0.439, -0.501, -0,489, P <0.05) C group and fasting blood glucose levels were positively correlation (R ~ 2 = 0.362,0.431, P <0.05); and fructosamine levels were positively correlated (R ~ 2 = 0.264,0.401, P <0.05); positively correlated with glycated hemoglobin (R ~ 2 = 0.206,0.375, P <0.05); conclusions: 1) patients with coronary artery disease serum soluble P-selectin levels were significantly higher than normal, which AMI patients was significantly higher when compared with patients with UA, indicating the presence of platelet UA and AMI were highly active state, suggestive of acute myocardial infarction compared with unstable angina platelet activation is more serious; their occurrence, development and platelet hyperactivity and thrombosis induced myocardial ischemia are closely related. 2) in patients with coronary heart disease total cholesterol, LDL cholesterol, and P-selectin levels were positively correlated, suggesting that cholesterol may stimulate vascular inflammation, activation of platelets. 3) coronary heart disease and diabetes, serum P-selectin levels were increased further than a simple CHD group, suggesting that platelet activation in blood glucose, serum P-selectin levels increased development may play a role.
