The Protective Effect of Splenectomy on the Hepatic Ischemia Reperfusion Injury

Abstract


Objective:To study the protective effect of splenectomy on the hepatic ischemia reperfusion in rats and the possible mechanism.Methods:Sixty male Wistar rats were divided into sham operation group, ischemia reperfusion group, ischemia reperfusion+splenectomy group. The hepatic artery and portal vein to the left and median liver lobes were occluded for 60mins; After the reperfusion for 3h、24h、48h、72h, the rats were killed while the liver and blood example were made. the levels of serum alanine amintransperase(ALT), aspartate aminotransferase(AST), total bilirubin(TBIL) were observed.The injury degree of the liver were determined by HE staining, the necrosis of the hepatocyte and infiltration of the neutrophilic granulocytes were determined by RT-PCR to detect the level of matrix metalloproteinases-9(MMP-9) mRNA.Results:After the reperfusion for 3h, the IR group and IR+SP group encountered injury. After the reperfusion for 24h, the level of ALT、AST、TBIL in serum in IR group and IR+SP group was the hignest, what’s more, the level of ALT、AST、TBIL in serum in IR+ST group was higher than in the IR group. After the reperfusion for 48/72h, the level of ALT、AST、TBIL in serum in IR group was higher than in the IR+SP group(P<0.05).The hepatocyte necrosis detection resuits:All the time in SH group were in the normal range. After the reperfusion for 3/24h,the level of Apoptotic index(AI) in group IR was lower than in the IR+ST group,but without statistical significance.However, after the reperfusion for 48/72h,the level of Apoptotic index(AI) in group IR was higher than in the IR+ST group,with statistical significance (P<0.05). The counter of the neutrophilic under high power lens results:After the reperfusion for 3/24h,the level of neutrophilic granulocytes in group IR was lower than in the IR+ST group,but without statistical significance.However, after the reperfusion for 48/72h,the level of neutrophilic granulocytes in group IR was higher than in the IR+ST group,with statistical significance(P<0.05). The HE staining results:All the time in SH group show hepatic lobules lined up in order, without significant tissue injury and neutrophil infiltration.There were local tissue ischemaia injury and neutrophil infiltration in IR group, After the reperfusion for 3h,the necrosis area was 30±2.6%; after the reperfusion for 24h,the necrosis area was 80±5.2%. After the reperfusion for 48h,the necrosis area was 60±4.1%; after the reperfusion for 72h,the necrosis area was 50±3.4% in IR group. IR+ST group:There were local tissue ischemaia injury and neutrophil infiltration,compared with IR group,without statistical significance. After the reperfusion for 48h,the necrosis area was 40±3.0%; after the reperfusion for 72h,the necrosis area was 35±2.7% in IR+ST group were lower than in IR group, with statistical significance (P<0.05). The electrophoresis results in the production of MMP-9 andβ-actin showed: all the groups with uniformity expression ofβ-actin mRNA, after the reperfusion for 48h, the levels of the MMP-9mRNA in IR+ST group were lower than in IR group, was 60.14±0.54%; after the reperfusion for 72h, the levels of the MMP-9mRNA in IR+ST group were lower than in IR group, was 56.10±0.42%, with statistical significance (P<0.05)Conclusion:1.Splenectomy shows the protective effect on the hepatic ischemia reperfusion injury, after the reperfusion for 48h, the protective effect was much more significant; 2.The mechanism of the protective effect on the hepatic ischemai reperfusion injury maybe involved in decreasing the number of the neutrophil and the level of the MMP-9 in liver tissues.
