Abstract

Background Although China's AIDS prevention and control work in recent years has made some progress, but because of the risk factors for the spread of AIDS transmission is still widespread, has brought great difficulties to AIDS prevention and control work. A very important risk factor is the increasing floating population in urban and rural areas, the high proportion of floating population in the AIDS patients, the flow of the high mobility of the population, the spread of AIDS provides opportunities. Floating population in the city, the construction site is an important migrant workers, the characteristics of the part. The crowd population base, strong liquidity, men accounted for the vast majority, the majority is young adults living in single-sex groups for a long time, in the edge of the city life, coupled with away from the family, a sense of law and lack of knowledge of STD and AIDS, China has become one of the major risk factors for HIV infection prevalence. At present, in the outskirts of the city less targeted construction workers to AIDS research. Therefore, the study of the construction workers of AIDS prevention, care and support, you can understand the behavioral characteristics of the population and infection status quo, to take effective intervention model to improve AIDS prevention knowledge of construction workers and condom use, and reducing HIV high-risk behavior, play a very important role in the prevention of HIV transmission. The goal of this study to understand the status quo of the Nanhui district construction workers on knowledge, attitudes and risk behaviors of HIV and STI; 2, assess of H1V and STI prevalence and impact factors; 3, on the basis of existing data and baseline survey data to improve and implementation of prevention and intervention programs; 4 to assess the effect of the intervention plan. Research content and methods The study was divided into two parts. The first part is the HIV and STI knowledge, attitudes, health services, risk behavior and the influencing factors of the investigation. The part three times, and 0 for the baseline survey, 3 and 6 months, respectively, to conduct a follow-up survey. Baseline survey, all participants completed a baseline questionnaire, and hematuria specimens. The blood samples will be used for the detection of HTV, herpes virus type II, and syphilis. The urine samples will be used for the detection of chlamydia and gonorrhea. The first three months, when the first follow-up survey; six months for a second follow-up survey. The second part is the implementation and evaluation of a behavior intervention plan. Behavior intervention plan, the end of the baseline survey carried out in three different types, low-intervention group distributing promotional pamphlets; in the intervention group also posted posters, panels and playback missionary DVD; addition of high-intervention group also carried out face-to-face interviews. 3 and 6 months follow-up to assess the effect of short-term and long-term interventions. The three intervention plan intervention point in the six sites were randomly selected. Research results of a baseline survey results (1) building site in Pudong Nanhui district highly gathering place for the floating population, the survey of 657 construction workers, mainly in young and middle-aged, most sexually active age, the average age of 38.9 ± 10.1 years; gender composition accounted for 90.4% of the male-dominated; educational level is generally at a lower level, junior middle school education (56.2%); married mostly married proportion of 84.2%; temporary residence to the crowd, mostly in site simple shed on site entertainment monotonous. (2) the construction workers in general have a certain degree of understanding of AIDS prevention knowledge, the higher rate of AIDS transmission problems answered correctly, such as the sharing of syringes spread of AIDS (66.2%), \AIDS (72.1%) \lower rate of correct answers, such as \whether the spread of AIDS (26.0%), mosquito bites whether the spread of AIDS \(3) construction workers to treat HIV and AIDS patients there is still a certain amount of discrimination. 72,90% of the people think that AIDS is frightening \Premarital sex, extramarital sex attitude is gradually tend to open. Were 13.5%, 11.8% for married men, married woman having an affair behavior agrees; 12.2% of people agree that can occur in commercial sex; 7.2% of construction workers for the MSM agrees. (4) the proportion of the construction workers to use condoms when having sex at a low level, the prevalence of AIDS prevention knowledge and condom use among unrelated phenomena. Married construction worker, having sex with their spouses, 50.6 percent never used condoms, only 7.4% of the people is always used, mainly due to the nearly 70 percent of construction workers feel no need, but also 16.8% of people without the use of condoms because no feeling. Unmarried construction worker, never use condoms accounted for 41.3%, to each use accounted for only 14.4%; mainly due to the 57.7% uncomfortable, it does not matter a share of 23.1%. (5) The investigation found no HIV-infected persons, identified two cases of syphilis prevalence was 0.3%; herpes virus infection; gonococcal infection in 2 cases; 13 cases of chlamydial infection, the prevalence was 2.0% ; 2, the intervention effect evaluation baseline survey number 657 people, including low-intervention group of 224 people, 209 people in the intervention group, high intervention groups 224; 475 first follow-up, follow-up rate was 72.3%, which is low-intervention group 154 152 people in the intervention group, high intervention group 169; second follow-up of 330 people, follow-up rate was 50.2%, which low-intervention group and 102 in the intervention group and 91 high intervention group of 137 people. (1) low-intervention group baseline survey, the first follow-up survey, the second follow-up investigation AIDS knowledge score was 6.46 ± 3.94, 11.6 ± 2.25, 12.55 ± 1.36 points; first follow-up and the second follow-up with baseline comparison, and the first follow-up compared with the second follow-up, 14 scores differ significantly, there is a statistically significant (p lt; 0.01). First follow-up compared to baseline AIDS-related attitudes have two perspectives positive answer change significantly, there is a statistically significant (p lt; 0.01); in sexual attitudes, for the first time, the second follow-up, respectively, at baseline comparison, as well as the first time compared with the second follow-up, there were no significant differences (P gt; 0.01), not statistically significant. Never married and at the end of the marriage by having sex without a condom, \(2) in the intervention group baseline survey, the first follow-up survey, the second follow-up investigation AIDS knowledge score was 6.31 ± 4.26, 12.37 ± 1.48, 12.23 ± 2 minutes; first follow-up and the second follow-up with baseline In comparison, 14 scores significant difference was statistically significant (p lt; 0.01). The first follow-up compared to the baseline AIDS-related attitudes, there is a view of a positive answer changes significantly; second follow-up compared with baseline, have two perspectives positive answer change significantly, with statistical significance (p lt; 0.01 ); sexual attitudes, first, second follow-up were compared to the baseline, as well as the first and the second follow-up comparison, there was no significant difference (P gt; 0.01) was not statistically significant. \(3) high-intervention group baseline survey, the first follow-up survey, the second follow-up investigation AIDS knowledge score was 7.05 ± 4.35 points, 12.65 ± 1.43, 12.4 ± 1.76 points; first follow-up and the second follow-up with baseline In comparison, 14 scores significant difference was statistically significant (p lt; 0.01). First follow-up compared to baseline AIDS-related attitudes have two perspectives positive answer change significantly; second follow-up compared to baseline, 4 views forward answer there were significant changes in the statistical significance (p lt; 0.01); sexual attitudes, the first follow-up compared with baseline, three concepts forward answer change, the second follow-up compared with baseline, four concepts forward answer change significantly, statistically significance (p lt; 0.01) the married person sex never use condoms and the occasional use of condoms, \; 0.01); unmarried sex never use condoms, and every time you use a condom, \. (4) three intervention effects compare three interventions played a huge role in raising AIDS knowledge score. Baseline survey, the group of construction workers between AIDS knowledge score was no statistical significance (P gt; 0.01), and the three groups score lower; display in the intervention group and intervention group after the intervention in the first follow-up investigation compared with low intervention, knowledge scores increased significantly (p lt; 0.01), but the difference between the high intervention group and in the intervention group was not statistically significant (P lt; 0.01); second follow-up survey showed three groups knowledge scores between significant difference (P lt; 0.01), but the three groups of knowledge score is still higher. The three types of intervention also played a positive role in changing AIDS-related attitudes. Baseline survey, the three groups of construction workers l AIDS-related attitudes and sexual attitudes were not statistically significant (P gt; 0.01); After the intervention, the first follow-up survey showed that in the intervention group and intervention group with low intervention group, have an attitude perspective positive change was statistically significant (p lt; 0.01); second follow-up survey results show that the high intervention group lower intervention group 3 personalized conceptual issues, there is a positive change, a significant difference (p LT; 0.01). The three interventions related behavior change interventions object AIDS has played a certain role, but the effect is not as obvious change of knowledge. For a married person to use condoms with their spouses in terms of intervention after the first follow-up investigation did not show some differences; second follow-up survey showed that \group produced a significant difference (p LT; 0.01). For the proportion of unmarried sexual partners use condoms after the intervention in the first follow-up survey showed a significant difference (p lt; 0.01) between the intervention group and a low proportion of \high intervention group and low-intervention group showed a significant difference (p lt; 0.01), while the second follow-up, there was no significant difference. Conclusion 1, three interventions for AIDS-related knowledge awareness have very significantly improve; treat HIV-infected people and AIDS patients attitude positive aspects of a more substantial improvement. 2, through the intervention, and AIDS-related knowledge awareness improved attitude towards people living with HIV and AIDS patients have also changed, but no significant changes in AIDS-related risk behaviors. Before and after the intervention, the proportion of married and unmarried people having sex often use condoms \before and after the intervention, condom use is still showing some positive changes in the proportion of married and unmarried sex \each time with \3, distributing promotional pamphlets, posters placed promotional display panels, player the missionaries DVD, face-to-face interviews, and other interventions are applied to improve the construction workers the knowledge of AIDS and STD awareness, as well as to enable them to treat HIV infection and AIDS The patient's attitude to positive change. 4, through the survey, verify the improvement of knowledge does not mean a change in behavior, the only lasting behavioral intervention can have a broad impact. The low, three different methods of intervention, whether that of a more suitable work, pending further research in the areas of construction workers in AIDS education and behavioral intervention; same time, needs to explore other more reasonable more effective behavior intervention model for AIDS interventions in the educational work carried out the construction workers and other types of crowd and good effectiveness to achieve full crowd containment and control the spread of AIDS and venereal disease spread.
