Abstract

Objective: This article by analysis of heme oxygenase -1 (heme oxygenase-1, HO-1) expression in peripheral blood mononuclear cells of type 2 diabetes and patients with arterial elasticity and compare the concentration of bilirubin, carboxyhemoglobin concentration different, to further explore the relationship between blood glucose, insulin, urine protein, arterial elasticity and other factors in vivo HO-1 expression levels. Methods: between January 2009 - July 2009 to my hospitalized patients admitted to hospital Cardiology, 85 cases of including simple hypertension 26, the 28 simplex type 2 diabetes, type 2 diabetes patients with hypertension 31 people. And normal control group of 25 people. The clinical diagnosis of hypertension in line with the 2003 WHO criteria, type 2 diabetes group to comply with the 1999 WHO diagnostic criteria. Exclude recent fatal coronary heart disease events in patients with severe liver disease, anemia and infection history, no history of heavy metal contacts; exclude the recent history of acute glomerulonephritis, diabetic nephropathy, and family history of nephropathy nothing more; exclude hyperthyroidism, aortic failure patients. Order to analyze the impact of different factors on HO-1, according to different factors may affect the HO-1 grouping. The first comparison group is glucose HO-1, the patients were divided into two groups: the first group to the normal glucose group, 24 males and 19 females; second group of hyperglycemia group, 26 males, 25 females; second comparison group Compare 24-hour urine protein HO-1, divided into two groups: the first group of patients for 24 hours the proteinuria less than 500mg/24 hours, 11 males and 8 females ; second group of patients for 24 hours proteinuria than 500mg/24 hours, 7 males and 5 females. Rule out the recent history of acute glomerulonephritis, diabetic nephropathy, and family history of nephropathy nothing more; third comparator group is the impact of the the insulin application of HO-1, were divided into two groups: the first group of insulin application group, male six cases of seven cases, female; second group of non-insulin application group, 11 males and 7 females; fourth comparison group compare arterial elasticity for HO-1, collected hypertension patients were divided into two groups : pulse pressure higher than 60mmHg as a group, that high pulse pressure group, 14 cases; pulse pressure lower than 60mmHg as a group, that is, low pulse pressure group, 12 cases. Patient blood specimens collected, extracted monocytes immunohistochemistry to evaluate the expression of HO-1 monocytes; patient's liver function, renal function, blood glucose, blood count, blood gas tests and at the same time. To observe through the four comparison groups to analyze glucose, insulin, urine protein, arterial elasticity of patients in vivo expression of HO-1. Results: 1 the first set of results: 51 cases of patients with normal glucose tolerance, peripheral blood mononuclear cells in HO-1 expression score mean of 1.05 ± 1.45; 59 cases of high blood sugar in patients peripheral venous blood mononuclear HO-1 expression in the cells ratings mean of 3.07 ± 1.98. HO-1 expression in mononuclear cells of the patients with hyperglycemia group ratings mean significantly higher than that of patients with normal glucose group (P lt; 0.01). The carbon monoxide concentration of the two groups of patients (carbon monoxide present in the body is a form of carboxyhemoglobin carboxyhemoglobin concentration by comparing the two groups of patients can be derived carbon monoxide concentration of the two groups of patients), bilirubin concentration comparison They are the product of HO-1 catalytic heme. As can be seen from the results, the average concentration of the outer periphery of the 26 cases of patients with normal glucose tolerance in arterial blood carboxyhemoglobin 1.2935 ± 0.28574 g / 1, total bilirubin concentration of 15.3423 ± 3.65526μmol / l; 59 cases of high the outer periphery of the blood glucose in patients the mean arterial blood carboxyhemoglobin concentration 1.3169 ± ± 0.36214g / 1, total bilirubin concentration of 14.7464 ± 4.83908μmol / l; through statistical analysis found that the two groups of patients with peripheral arterial carbon the oxygen hemoglobin concentration and total bilirubin concentration was no significant difference (P gt; 0.05). 2 The results of the second comparison group: 12 patients with high proteinuria in patients with peripheral blood mononuclear cells of HO-1 expression score mean of 3.1667 ± ± 1.4668; 19 patients with low proteinuria in patients with peripheral blood mononuclear cells HO-1 expression score mean is 2.8947 ± 1.2425. Between the two groups, there is no statistically significant difference (P gt; 0.05). Third comparison group results: 13 cases of insulin treatment group patients with peripheral blood mononuclear cells of HO-1 expression score mean of 2.3846 ± 0.6504; 18 patients with non-insulin medication group patients with peripheral blood mononuclear cells of HO -1 expression score number to 3.44444 ± 1.5038. There are differences between the two groups (P lt; 0.05), HO-1 expression in the peripheral venous blood mononuclear cells of patients with non-insulin medication group stronger than the insulin treatment group. The average concentration of total bilirubin were compared in patients with peripheral arterial carboxyhemoglobin, the results can be found in the average concentration of total bilirubin in 13 patients with insulin treatment group of patients with peripheral arterial as 15.4077 ± 3.51697μmol / l, the average concentration of carboxyhemoglobin the 1.4331 ± the ± 0.42619g /; average concentration of the outer periphery of the arterial blood of 18 patients with non-insulin medication patients total bilirubin 14.5500 ± 3.48193μmol / l, the average concentration of carboxyhemoglobin 1.29 ± 0.43090 g / 1. The average concentration of total bilirubin and carboxyhemoglobin insulin treatment group with non-insulin medication group, patients with peripheral arterial There was no significant difference (P gt; 0.05). 4. The fourth comparator group results: 14 cases of patients with high pulse pressure group peripheral venous blood monocytes HO-1 expression score mean is 1.6429 ± 0.63332; poor group of 12 patients with low pulse pressure in patients with peripheral blood mononuclear HO-1 expression in the cells ratings mean 1.6667 ± 0.65134. HO-1 expression scores mean high pulse pressure group with low pulse pressure difference between groups in patients with peripheral blood mononuclear cells, no significant difference between different (P gt; 0.05). Conclusion: The blood sugar induced HO-1 expression levels, but did not benefit from the HO-1 expression is elevated. HO-1 expression and the progress of diabetes is no correlation. The use of insulin on the expression of HO-1 was no effect, and therefore the expression level of HO-1 can be indirectly reflects the therapeutic effects of insulin. Arterial elasticity change HO-1 expression. I hope to lay the foundation for future clinical and basic research.
