Abstract


Objective: To survey the incidence of Jincheng Coal Group General Hospital ICU with acute respiratory distress syndrome (ARDS) , the primary disease , mortality and its influencing factors . Methods : According to the Chinese Medical Association of Critical Care Medicine Branch acute lung injury / acute respiratory distress syndrome diagnosis and treatment guidelines ( 2006 ) in the diagnostic criteria , follow the 1994 Joint Meeting of the European and American ARDS developed ARDS diagnostic criteria , retrospective survey 2004-2009 Jincheng Coal Group General Hospital ICU patients with ARDS , the primary disease , the yearly incidence of dead and its influencing factors . Results :2004.1 2009.12 Jincheng Coal Group General Hospital ICU were treated 1714 cases of critically ill patients , 60 patients met the diagnostic criteria of ARDS , accounting for 3.5% of the same period in ICU types of critically ill patients admitted to . Lung disease caused by the ARDS ( Intrapulmonary type ARDS) of 16 cases ( 26.7 %) , 10 patients died , the fatality rate was 62.5% . Extrapulmonary disease induced ARDS ( lung appearance ARDS) a total of 44 cases ( 73.3 % ) , 21 cases of death , mortality was 47.1% . Intrapulmonary type ARDS mortality was significantly higher than the lung appearance (x2 = 13.261 , P lt ; 0.01) . Primary disease as sepsis, 13 patients ( 21.7 % ) and pneumonia in 10 patients ( 16.7 % ) , major surgery, severe pancreatitis (5%) , multiple trauma (25%) . Death in 31 cases , 60 cases of patients with ARDS lung disease deaths in 10 patients ( 32.3 % ) , extrapulmonary disease death in 21 cases ( 67.7 %) . ARDS mortality rate was 51.67% . Septic shock is a major cause of death , only 29.03% died of respiratory failure . 6 years, mortality did not change. Conclusion: 1.6 years ARDS average incidence rate of 3.5% , the lowest in 2007 (2.97 %) from 2004 ( 3.17% ) to 2009 ( 4.06% ) showed a slow upward trend . To 2004 as the base year , 2009 ARDS onset of the growth rate of ( fixed base ratio ) of 100% from 2007 to 2009 , the incidence of the chain increases every year . 2.6 years total mortality of 51.67% ; intrapulmonary type ARDS mortality is significantly higher than the lung appearance of ARDS , mortality was no significant change in six years . The 2004 -2009 mortality the ring than no downward trend year by year . 3.ARDS patients did not die of simple respiratory failure , ARDS majority of patients died of septic shock due to mechanical ventilatory support technological progress ; 4. Occurrence of septic shock or multi- organ failure (MOF) , the mortality was significantly increase.
