The Study on the Relationship between NF-kappaB Aberrant and the Risks of Oral Squamous Cell Carcinoma Occurring on Areca Chewers

Abstract


Objective: (1) To determine the expression of aberrant NF-κB in oral submucous fibrosis(OSF) and oral squamous cell carcinoma (OSCC) patients occurring on areca chewers.(2) To study correlation if any between aberrant NF-κB and risks in OSF and OSCC occurring on areca chewers.Methods: Study Design consists of biopsy specimens of 21cases of normal control and 76 cases of OSF and 47 cases of OSF with OSCC were subjected for staining by immunohistochemistry for NF-κB using SP visualization .Clinical details along with habits were recorded and the data analyzed with t-test and chi-square test.Results: (1) The study reveals 1 case of normal control an 40 cases of OSF and 40 cases of OSF with OSCC were positive for NF-KBp65 expression. (2) In normal、the prophase of OSF、the metaphase of OSF、the advanced stage of OSF,oral squamous cell cairnoma with OSF ,the positive rate of NF-κBp65 was 4.76%、33.33%、60.61%、63.12% and 85.11 %,respectively.(there were siganificant differences between normal and the prophase of OSF, the prophase of OSF and the metaphase of OSF, the advanced stage of OSF and OSCC with OSF, P<0.05).(3) The cigarette and drinking consumption and age in OSF with OSCC was significantly higher than the OSF without OSCC .Chi-square test analysis was used to identify the risk factors. Smoking and drinking habits showed a significant effect, and the odds ratios were 2.61 in smoking and 3.76 in alcohol consumption.(4) A progressive increase in NF-κBp65 expression in OSF and OSF with OSCC following the increase of the age;Smoking habits showed a significant effect in NF-κBp65 expression in OSF and OSF with OSCC ,but there was no significant effect for alcohol consumption.Conclusion: Higher percentage of NF-κBp65 positive expression in OSF with OSCC cases when compared to oral submucous fiborsis cases. It suggests NF-κB expression may correlate with increase in malignant transformation for oral submucous fiborsis. Age ,smoking and drinking habits could be risk factors associated with an increased risk of malignant transformation and coexistence with oral cancer in OSF patients. Both age and smoking habits had a significant role in NF-κB positive expression.
