Abstract

Objective The present study investigated the relationships among the catechol-o-methytranserase(COMT)gene polymorphisms、psychological factors,including anxiety、depression、personality,acute post-operative pain and the efficacy of fentanly。Methods Ninety ASAⅠ-Ⅲinpatiens undergoing abdominal elective surgery between 20 and 75 yeas old completed the Self-Rating Depression Scale(SDS) inventory、the State-Trait Anxiety Inventory(STAI)and Eysenck Personality Questionaire(EPQ)inventory before the day of surgery.Standard intravenous general anesthesia、surgery and intravenous patient-controlled analgesia(IV-PCA) was conducted.Postoperative ratings of pain intensity,opiod consumption、number of PCA injections and demands、blood pressure、heart rate and saturation of blood oxygen were recorded after surgery on the ward。The degree of pain intensity was evaluated by a visual analogue scale(VAS,1-10).For COMT genotyping,4 ml of venous blood from each subject was collected.Results The pain intensity,total analgesic consumption in 24h and4 8h were signicantly related to preoperative trait-anxity,.state-anxity and depression (p=0.000,0.001).Fentanly consumption in 48h was signicantly related to age(p=0.018,0.008).But theCOMT Val158Met polymorphisms did not show association with opiod consumption and psychological factors.Conclusions The psychological factors were signicantly associated with pain intensity and total analgesic consumption after surgery.
