Abstract


Purpose retrospective study in our hospital in recent years TB cases in the surgical treatment of 98 cases of thoracic and lumbosacral summary of spinal tuberculosis advantages and disadvantages of each technique and indications . Methods From January 2001 to June 2007 in our hospital , surgical treatment of thoracic , lumbar and sacral spinal tuberculosis with 126 cases , 98 cases were effective follow-up . 53 cases were male and 45 females ; aged 16 to 76 , with an average of 39.6 years old ; history of 3 to 39 months , with an average of 15 months . According to the different surgical group : A group ( 21 cases ) using simple debridement B group (16 patients ) with anterior debridement and bone grafting and internal fixation , the C group (49 patients ) with an anterior focus Clear posterior interbody fusion and fixation ; D group ( 12 patients) with isolated posterior interbody fusion and pedicle screw fixation surgery . The results the incision healed of all patients , no chronic sinus formation . In addition to the 28 cases were lost , and the remaining 98 cases 6 months to 72 months follow - up , an average of 33 months . 75 cases of kyphosis , the average kyphosis correction was 18.3 degrees , the loss of the follow-up period of 3 to 5 degrees . In addition to the two cases of group A second surgery to cure , and the remaining patients were cured . 45 cases of neurological damage in patients achieve significant improvement , with an average improvement of 1.33 . The pairwise comparison χ2 test , the A / D group surgery time and blood loss was significantly less than in the group B / C group ( P lt; 0.05 ) . Conclusion on the basis of effective anti-TB chemotherapy , spinal stability is important in the treatment of spinal tuberculosis . Can effectively stabilize the spine surgical procedures to achieve a satisfactory clinical efficacy guarantee . Spinal cord compression , paraplegia and spinal instability by the need for surgical intervention . The obvious way forward is caused by compression of the nerve damage in patients for anterior debridement . Bone defects less than two segments is feasible anterior fixation ; of multiple vertebral tuberculosis or jump tuberculosis and lumbosacral tuberculosis is the best line of the posterior pedicle fixation . Pure posterior pedicle screw fixation and posterolateral spinal fusion can effectively stabilize the spine prevent tardive kyphosis , treatment less bone destruction , less nerve damage of spinal tuberculosis with light surgical trauma than the technology easy to carry out the advantages .
