Abstract

Objective:Functional dyspepsia(FD)is one of the most common clinical syndrome,the prevalence of FD has been noted to vary between 11%-29.2%.The quality of life of patiens had descended conspicuously,the medical resources had been wasted.The pathogenesis of FD has not been elicited.At present,it has been considered that many ingredients including abnormal motility of upper gastrointestinal tract,visceral hypersensitivity,excessive gastric acid secretion,infection of Helicobacter pylori, factors of psychology,society and diet may be involved in its development separately or complexly.Ghrelin is a growth-hormone-releasing peptide which has twenty eight amino acids.Leptin is a hormone which be encoded from obese gene and secreted by adipose cell.They were two kinds of brain-gut peptide which have been discovered recently.Their effects on gastrointestinal tract are recognized gradually.The factors of psychology play a significant role in the development of FD,especially anxiety and depression.We aimed to investigate the level of serum Ghrelin and Leptin,the condition of anxiety and depression in FD.We hope that this disease can be recognized gradually to elucidate the pathogenesis and give the effective treatments on FD.Methods:Forty four patiens with FD were divided into two groups:one group is postprandial distress syndrome(PDS)including twenty six patiens,the other group is epigastric pain syndrome(EPS),including eighteen patients in it.Meanwhile,thirty nine normal person were regarded as controls.Enzyme linked immunosorbent assay was used to examine the level of serum Ghrelin and radioimmunosorbent assay was used to examine the level of serum leptin.We use Self-Rating Depression Scale(SDS) and Self-Rating Anxiety Scale(SAS)to evaluate the condition of anxiety and depression.The results were be analyzed by Independent-sample T Test,One way ANOVA,spearman rank correlation,chi-square test respectively.Results:1.The level of serum Ghrelin in group FD was significantly lower than that in normal control(P＜0.01).The level of serum Ghrelin in group PDS was significantly lower than that in normal control(P＜0.01)and group EPS (P＜0.05).But no significant difference was found between the level of serum Ghrelin in group PDS and group EPS(P＞0.05).2.The level of serum leptin in group FD was significantly lower than that in normal control(P＜0.05).The level of serum leptin in group PDS was significantly lower than that in normal control(P＜0.01)and group EPS (P＜0.05).But no significant difference was found between the level of serum leptin in group PDS and group EPS(P＞0.05).3.There were significant correlations with the alteration of serum leptin and Ghrelin in group PDS.But in group EPS,there were no correlations with the alteration of serum leptin and Ghrelin.4.In group FD and normal control,leptin/Ghrelin was no significant difference (P＞0.05).In group PDS and group EPS and normal control,leptin/Ghrelin was no significant difference too(P＞0.05).5.In group FD,group PDS and group EPS,the SAS score,the SDS score,the ratio of anxiety,the ratio of depression,the ratio of together with anxiety and depression which were significantly higher than that in normal control.But no significant difference were found between the group PDS and group EPS (P＞0.05).There were significant correlations with the alteration of SAS score and SDS score.And there were significant correlations with the FD and SAS score and SDS score.6.In group FD,group PDS and group EPS,there were no significant correlations with the level of serum Ghrelin,the level of serum leptin and the SAS score,the SDS score(P＞0.05).Conclusion:1.The level of serum Ghrelin and leptin in group FD was decreased,the level of serum Ghrelin and leptin in group PDS was decreased.There were significant correlations with the alteration of serum leptin and Ghrelin in group PDS.These results suggest that the alteration of serum leptin and Ghrelin may play a promotional role in the development of FD.It may induce the symptoms of early satiation and postprandial fullness2.The level of serum Ghrelin and leptin in group EPS was not significantly changed,it suggest that EPS and the alteration of serum leptin and Ghrelin may have no obvious relation.3.The SAS score and the SDS score in patients of FD,PDS and EPS were all increased,it suggest that anxiety and depression may play a promotional role in the development of FD.If the corresponding treatment is given for them,the patiens with anxiety and depression would get better.
