Abstract


Objective: To investigate obstructive sleep apnea hypopnea syndrome (OSAHS) patients with plasma tissue-type plasminogen activator (t-PA), plasminogen activator inhibitor -1 (PAI-1), platelet α particles membrane protein level of -140 (GMP-140) and nasal continuous positive airway pressure (nCPAP) treatment before and after the change and its clinical significance. Methods: by polysomnography (PSG) confirmed 30 cases of patients with OSAHS and 30 cases of normal control group, OSAHS patients to receive one week of nCPAP treatment was measured by enzyme-linked immunosorbent assay (ELISA) plasma t- PA, PAI-1 and GMP-140 level. Compare the OSAHS group before and after treatment with the control group, the difference of the front and back indicators of with OSAHS group nCPAP treatment; analysis OSAHS patients with t-PA, PAI-1, of GMP-140 and PSG monitoring indicators of relevance. Results: 1.t-PA: before treatment in OSAHS group (8.69 ± 1.19) ng / ml was significantly lower than the control group (16.71 ± 3.37) ng / ml (P lt; 0.01); treatment in OSAHS group (13.96 ± 2.98) ng / ml is still lower than the control group (P lt; 0.05); plasma t-PA levels in OSAHS group after treatment than before treatment was significantly higher (P lt; 0.01); 2.PAI-1: OSAHS group before treatment (87.76 ± 9.93 ) ng / ml, after treatment (54.93 ± 10.94) ng / ml was significantly higher than that in the control group (27.00 ± 8.09) ng / ml (P lt; 0.01); plasma PAI-1 levels in OSAHS group therapy treatment significantly reduced (P lt; 0.01); 3.GMP-140: OSAHS group before treatment (52.36 ± 8.16) ng / ml after treatment (37.75 ± 5.72) ng / ml was significantly higher than that in the control group (13.47 ± 1.73) ng / ml plasma GMP-140 level before treatment (P lt; 0.01); in OSAHS group therapy significantly reduced (P lt; 0.01); 4.PSG monitoring indicators: (1) AHI: OSAHS treatment group (33.67 ± 22.43) times / h was significantly higher than that in the control group (2.17 ± 1.28) / h (P lt; 0.01); significantly lower in OSAHS group after treatment AHI (1.10 ± 0.98) times / h than before treatment (P lt; 0.01); (2) the minimum SaO2: before treatment in OSAHS group (67.30 ± 14.01)% was significantly lower than the control group (85.13 ± 3.66)% (P lt; 0.01); lowest SaO2 in OSAHS group after treatment (84.47 ± 4.27)% treatment was significantly higher (P lt; 0.01); (3) 90% oxygen desaturation index: before treatment in OSAHS group (21.32 ± 17.70) was significantly higher than that in the control group (2.18 ± 2.19) (P lt; 0.01); less than 90% after treatment in OSAHS group oxygen desaturation index (2.81 ± 2.51) significantly lower than before treatment (P lt; 0.01); 5. (1) OSAHS patients with plasma t-PA level of their AHI less than 90% oxygen desaturation index was negatively correlated ( r = -0.674, P lt; 0.01; r = -0.616, P lt; 0.01); their lowest SaO2 was positively correlated (r = 0.604, P lt; 0.01); (2) OSAHS patients plasma PAI-1 levels of their AHI less than 90% oxygen desaturation index showed a positive correlation (r = 0.725, P lt; 0.01; r = 0.688, P lt; 0.01); their lowest SaO2 was negatively correlated (r = -0.613, P lt; 0.01); ( 3) OSAHS patients plasma GMP-140 level of their AHI less than 90% oxygen desaturation index were positively correlated (r = 0.869, P lt; 0.01; r = 0.824, P lt; 0.01); their lowest SaO2 was negatively correlated ( r = -0.587, P lt; 0.01). CONCLUSION: OSAHS patients with reduced fibrinolytic activity and platelet activation, they may be involved in the occurrence and development of OSAHS patients with cardiovascular and cerebrovascular thrombotic complications. Lower ② OSAHS plasma t-PA levels, PAI-1, GMP-140 levels increased with hypoxemia, AHI is closely related. ③ nCPAP treatment can improve OSAHS patients AHI, hypoxemia, and thus make the elevated level of t-PA, PAI-1, GMP-140 levels decreased, and thus play a therapeutic role.
