The Microinflammation State in Non-dialysis Patients of Chronic Kidney Disease

Abstract


Objectives To investigate the serum level of interleukin-6 (IL-6)、High sensitive C-reactive protein(hs-CRP)、Iipoprotein(a)[LP(a)] in patients with chronic renal disease and analyzed the correlation between the microinflammation state and the risk factor of cardiovascular disease.Methods 100 patients with chronic renal disease (CKD group ) and 25 healthy people (control group) included in the present study. Patient’s glomerular filtration rate (eGFR) was assessed by MDRD formula. According to the standard of NKF-K/DOQI proposed, 100 patients of CKD group were divided into CKD1-5 stage (5 groups). Serum level of IL-6、hs-CRP、LP(a) was determined by ELISE assay. Some related clinical indicators like Scr、BUN、UA、APOA、APOB、LDL、HDL、TG、TC were detected at the same time. To compare with the serum IL-6、hs-CRP、LP(a) level in different disease groups. The relationships were analyzed between the serum IL-6、hs-CRP、LP(a) level and renal function and other clinical indicators.Results1. The serum level of hs-CRP (3.54±2.57 mg/l vs 1.85±1.36 mg/l), IL-6(2.10±1.29 pg/ml vs 1.53±0.93 pg/ml), LP(a) (326.34±124.39 mg/l vs 152.32±63.46 mg/l) was significantly higher in CKD group than that in control group (P<0.05).2. In CKD group, patients’serum IL-6 level was significantly higher in patients with grade4, 5 renal function than that patients with gradel 1,2,3 (P<0.05). Patients’serum IL-6 was significantly associated with APOA. Scr and BUN (P<0.05).3. The serum hs-CRP level was significantly higher in patients with grade 3,4,5 renal function than that patients with grade l,2 (P<0.05). The serum hs-CRP was significantly associated with Scr,BUN and UA (P<0.05). 4. The serum LP(a)level was significantly higher in patients with grade 3,4,5 renal function than that patients with grade l,2 (P<0.05).The serum LP(a)was significantly associated with Scr, BUN,and UA (P<0.05).5. Multiple stepwise regressive analysis showed that increased hs-CRP in CKD patients was closely related to the levels of Scr.Conclusion1. The elevated serum IL-6、hs- CRP、LP(a) level increase with the renal hypofunction periods in patients with chronic kidney disease.2. Microinflammation factors such as Hs-CRP, IL-6, LP(a), has a higher predictive value than traditional factors of cardiovascular disease for patients with chronic kidney disease, that my be supply valuable for the diagnostic and identifying high-risk patient with cardiovascular disease in CKD patients.
