Abstract


Objective: To study the substance P (substance P, SP) and its receptor (substance P receptor, SPR) in irritable bowel syndrome (irritable bowel syndrome, IBS), the development of the role, to explore the pathogenesis of IBS. 40: clinical symptoms Rome II criteria for IBS patients, including diarrhea-predominant group (20 cases) and constipation-predominant group (20 cases), 15 asymptomatic controls. Colonoscopy and pathology of all patients found no pathological changes in the colonic mucosa, colonoscopy take sigmoid colon mucosal tissue, SP immunohistochemical staining analysis and SPR mRNA in situ hybridization analysis; random add to take the which 8 patients sigmoid colon Organization observe the changes of the colonic mucosa cell ultrastructure. The statistical analysis of the expression of SP and SPR diarrhea and constipation IBS patients with intestinal mucosa. Results: ① colonic mucosa layer SP expression: expression of SP are mainly located in the colonic mucosa epithelial cells, the cells of the intestinal glands, lamina propria mast cells, neutrophils, lymphocytes and SP immunoreactive fibers, etc.. Diarrhea-predominant IBS group, the expression of constipation-predominant IBS group and control group patients SP intensity difference was significant (P lt; 0.01); pairwise comparison, diarrhea-predominant IBS group SP expression was significantly higher than that in the control group, the difference was significant (P lt; 0.017), while no significant difference between patients with constipation-predominant IBS SP expression and control group (P GT; 0.05), but the lamina propria mast cells, neutrophils, lymphocytes, and SP immunoreactive fibers etc. expression was significantly higher than the expression of the epithelial cells and glandular cells, the difference was significant (P lt; 0.017); diarrhea-predominant IBS group SP expression was significantly stronger than the constipation-predominant IBS group (P lt; 0.017). Constipation-predominant IBS group colonic mucosa connective tissue SP expression was significantly stronger than the epithelial and glandular cells (P lt; 0.017). (2) Results expression of mucosa SPR mRNA situation: diarrhea-predominant IBS group, constipation-predominant IBS group and the control group SPR mRNA expression difference is very significant meaning (P lt; 0.01); pairwise comparison, the expression of the diarrhea-predominant IBS group was significantly stronger in control group and constipation-predominant IBS group (P lt; 0.017), constipation-predominant IBS group was significantly worse than that of the control group (P lt; 0.017). (3) Electron microscopy found that IBS patients with diarrhea Junction intestinal epithelial cell membrane integrity, cell morphology no abnormal mucosa goblet cells, plasma cells, mast cells and nerve within the ultrastructure of the secretory cells are visible to a large number of secretory granules, vacuoles, crude endoplasmic reticulum, showing the active function and secretion of strong state.
