Non-reamed Versus Reamed Intramedullary Nailing for Tibial Fractures in Adult: A Systematic Review

Abstract


Objective: the non- reamed reamed intramedullary nail fixation of tibial shaft fractures in adults to evaluate the efficacy . Method : computer searches of MEDLINE ( ??1966 to 2005.7 ) , EMBASE ( 1966 to 2005.7 ) , the Cochrane Library ( 2005 ) and the Chinese Biomedical Database ( 1979 to 2005.7 ) , manual searches in English orthopedic magazine . Collect all to unreamed with reamed intramedullary nail fixation compared to the treatment of adult tibial shaft fractures clinical randomized controlled and quasi-randomized controlled trials included in the study and evaluation of methodological quality . The Cochrane Collaboration statistical software RevMan4.2.7. Results: A total of unreamed and reamed intramedullary nail fixation compared to the treatment of adult tibial shaft fractures 4 randomized controlled trials and quasi-randomized controlled trials 1 . Meta-analysis showed that non- reamed reamed intramedullary nailing for the treatment of adult tibial shaft fractures compared fracture healing rate elevated [ RR = 1.87, 95 % CI ( 1.20,2.91 ) , P = 0.006 ] ; implant failure was significantly higher [ RR = 2.23, 95 % CI ( 1.49,3.34 ) , P < 0.0001 ] ; fracture healing time [ WMD = 9.00,95 % CI ( 3.19,14.81 ) , P = 0.002 ] . Conclusion: The non- reamed intramedullary nailing for the treatment of adult tibial shaft fractures compared with reamed intramedullary nailing nonunion higher implant failure is more common . Reamed or not the incidence of postoperative infections , malunion rate , the compartment syndrome two groups showed no significant difference in the incidence of . In severe open fractures ( Gustilo Grade â…˘ C ) , combined with multiple hair damage , reamed or not the impact of these indicators to be further studied .
