Abstract

Background and purpose of the uterus, endometriosis (endometriosis, EMs, referred to within the endometriosis) is a common gynecological diseases in women of childbearing age, there is a rising trend in the incidence of major clinical manifestations of infertility, dysmenorrhea, painful intercourse chronic pelvic pain and other symptoms, a serious impact on the physical and mental health and quality of life of women of childbearing age. The endometriosis research focuses on its pathogenesis, basic research and the pain of endometriosis rarely reported. Therefore, the importance of the study of endometriosis with pain relationship to strengthen the treatment of pain, medical workers has become an important research topic. Oxytocin receptor (oxytocin receptor, OTR) is a G protein coupled receptor (G protein-coupled receptor, GPCR) superfamily, is widely distributed within the central nervous system and peripheral tissue, different tissue cells oxytocin receptors and oxytocin, can play a role in a variety of signal transduction pathways. Oxytocin receptor mostly confined in obstetrics, in recent years, with the development of molecular biology techniques, many scholars have found that oxytocin receptors in the occurrence of the tumor within different psychosis development are play a role. Estrogen receptor (estrogen receptor, ER) belongs to the family of nuclear receptors in hormone connection, transcriptional regulation, cell interactions play an important role in both. Estrogen binding to estrogen receptors, may be on the level of the receptor facilitation of phospholipase C activity, time, dose-dependent regulation of oxytocin receptors. By immunohistochemical methods of this study was to observe the OTR, the expression of ER in the EMs, analysis of the role in the pathogenesis of endometriosis, including them and their pain, and their relationship with endometriosis pain for endometriosis pain treatment provides a theoretical basis. Materials and methods. Study group January 2005 to April 2006, the Third Affiliated Hospital of Zhengzhou University, which needed a hysterectomy or excision of the ectopic lesions inpatients suffering from endometriosis endometriosis group of 50 patients (postoperative specimens confirmed by pathological examination visible the ectopic gland epithelial and mesenchymal cells), the average age of 41.30 ± 5.0 years, eutopic endometrium of 40 cases, 10 cases of ectopic endometrium. Select the same period as a control group of 30 patients due to the septate uterus mediastinal inpatient resection (excluding intrauterine adhesions, postoperative specimens confirmed by pathological examination normal endometrium), with an average age of 40.10 ± 5.0 years, similar to the endometriosis group (P> 0.05). While the endometriosis eutopic endometrium group patients according to the presence or absence of pain painless group of 17 patients and pain in 23 patients. In addition, the pain visual analog scale (VAS) pain intensity, degree of pain, endometriosis eutopic endometrial pain patients were divided into three groups: 11 patients with mild, moderate group of seven cases, five cases of severe group. The normal menstrual cycle patients in each group, six months before surgery were not used hormone drugs, not the risk of endometrial cancer, ovarian cancer, dysfunctional uterine bleeding, breast cancer and other disease closely related to hormone levels. 2. The experimental method using immunohistochemistry (SP method) detection of oxytocin receptor, the estrogen receptor in the eutopic endometrium of endometriosis, ectopic endometrium and normal endometrium expression. 3. Pain score based on the visual analogue scale method (VAS), Pain Society of the Chinese Medical Association Producer VAS card, the card center engraved with numbers can slide the cursor in the the 10cm long term on both ends, respectively the painless (0) and the most intense Pain (10), patients face side without scale, the cursor on the site was the most representative of the degree of pain, the doctor facing scale side, and to record the degree of pain. The pain evaluation criteria: mild pain 1-4cm (such as discomfort, the blunt pain); moderate pain 5-7cm (such as tenderness and tenderness); 8-10cm (moderate to severe pain, such as disruption to normal life, can not control). 4. Statistical analysis of the data obtained are ((?) ± s) univariate analysis of variance test using SPSS11.5 software package for statistical analysis, among the three groups, significant test level a value is set to 0.05, two The analysis of the correlation between the indicators adopted person product moment correlation calculations. Results 1. OTR protein-positive substance mainly localized on the cell membrane of the epithelial cells of the endometrium, a small amount of expression in the cytoplasm, stromal cells do not express. EMs eutopic endometrium, of ectopic endometrium OTR protein expression were higher contrast differences were statistically significant (p <0.05); higher than the the OTR protein levels in the pain group of endometriosis painless group, compared the difference was statistically significant (p <0.05): the OTR positive gray value of the protein in the EMs severe pain group organization in the highest and lowest gray values ??in the mild pain group, among the three groups differences were significant (p <0.05) . 2. The ER protein-positive substance was mainly localized in endometrial glandular epithelial cell nuclei with EMs the eutopic endometrial ER protein expression higher than that of the control group, the difference was statistically significant (p <0.05); pain group of endometriosis and painless group expression of the difference was statistically significant (p <0.05); ER protein positive EMs severe pain group organization grayscale value of the highest and lowest gray values ??in the mild pain group, the differences among the three groups were significantly significant (p <0.05 ). 3. Eutopic endometrium, the of OTR expression level of ER expression levels were positively correlated relationship (r = 0.935, P <0.001), in the expression of ectopic endometrium and the control group in both correlation (P = 0.726 and P = 0.152). Conclusion 1. OTR, ER endometrial eutopic endometrium of high expression showed that its abnormalities may be closely related to the development of the disease. 2. OTR, ER showed high expression of endometriosis pain organizations and organization of moderate to severe pain that may be involved in the development of disease with pain, at the same time may be a correlation between the degree of pain and endometriosis. 3. Eutopic endometrium, OTR, ER has a high degree of correlation, indicate that there may be some connection, and jointly promote the development of endometriosis and pain.
