Changes of Thyroid Function Effect on Outcomes of Pregnancy and Expression of Iodothyronine Deiodinase

Thyroid function during pregnancy and autoimmune status change to the adverse effects on pregnant women and their offspring has received extensive attention , pregnancy whether it should become a hot topic in the routine screening of thyroid function and antibody . This topic for prenatal care in hospital obstetrics at the period from March 2006 to February 2007 , 632 cases of pregnant women who had no history of thyroid disease , thyroid function screening and obstetric complications observed , in some cases placenta, uterine tissue iodide thyroid thyroxine deiodinase detection . The results show that the downward trend with increasing gestational age , pregnant women, FT3, FT4 level , but no significant differences ( P gt ; 0.05) . In the past this group of pregnant women with no thyroid history , clinical and subclinical thyroid disease prevalence rate was 11.87% , subclinical hyperthyroidism was 0.16% (n = 1 ) , subclinical hypothyroidism was 6.80% ( 43 cases ) , clinical A reduced to 1.90% ( 12 cases ) , low Hypothyroxinemia of 3.01% ( 19 cases ) ; 7.75% TPOAb -positive pregnant women . In subclinical hypothyroidism in pregnant women fetal distress prevalence was 11.63% ; the group Hypothyroxinemia , intrauterine growth retardation prevalence was 10.53% , higher than the normal control group . Normal thyroid function , TPOAb positive group intrauterine growth retardation prevalence was 4.65% , only 0.58% and TPOAb negative . Maternal TSH levels associated with fetal distress than patients without complications , premature than those of patients without complications dropped . Placenta , uterus deiodinase detected by RT-PCR method , found that pregnant women, uncontrolled hypothyroidism Ⅱ type deiodinase enhanced expression of type III deiodinase weakening ; previous history , but prenatal control with no significant differences in normal control . The results of this study suggest that abnormal thyroid function and autoimmune state is closely related with a variety of obstetric complications . Uterus , placenta deiodinase may be related to the mother and fetus between the level of thyroid hormone regulation . Thyroid function during pregnancy and antibody screening helps early detection including clinical , especially subclinical thyroid disease , and provide timely treatment to improve obstetric prognosis .
