Objective To compare the total hip arthroplasty acetabular whether the screw effect in terms of stability. Changes observed postoperative weight-bearing the two groups acetabular position. Uncemented acetabular cup 60 total hip arthroplasty patients followed up for the period from 2008 to 2010, a total of 62 hips. The average follow-up time was 24 months. Acetabular screws were divided into two groups according to whether the implantation surgery. : A group screw group and 32 hips in group B 30 hip screw group. Assessment of radiographic change and Harris score were compared between the two groups acetabular stability and clinical effect. The results observed X-ray acetabular periprosthetic bone imaging changes within 24 months. 62 hip cases, 32 cases 1-2 screws implanted in the the acetabular above C1 District, 30 patients without implanted screw. Periprosthetic obvious gap by assessing subsequent bone sclerosis and radiolucent line appears time to assess the effect of surgery. Group B, six days after the line X-ray photographs, often seen in the screw group C2 District gap (7 hips, 23.33%), the gap completely disappeared after 24 months. Group A, six days after the C2 zone is less gap (1 hip, 3.12%). Screw group, one of the 15 hips (46.88%) after five months C1 District osteosclerosis. With the advent of osteosclerosis between 5-24 months, 10 hips (31.25%) in the C3 region radiolucent line. 8 (25.00%) of the hip after 16 months C1 District radiolucent line. In group B, C1 and C3 rarely osteosclerosis and radiolucent line. By comparing two groups of postoperative abduction angle and hip acetabulum rotation center bit value changes assess the effect of surgery. The two groups of patients after the abduction angle and rotation center bit value changes the difference was not statistically significant (P gt; 0.05). The average Harris hip score of the follow-up the final group A (93.7 ± 2.0) points in the B group, the average Harris hip score (95.3 ± 1.5) points, the difference was not statistically significant. To the end of the follow-up the patient prosthesis dislocation. Conclusion group screw with screw group after acetabular position and clinical effect, there was no significant difference, but no the screw groups around the acetabular cup less image changes occurred, implanted total hip arthroplasty acetabular screw is not must. Total hip arthroplasty, by applying pressure with technology, to obtain the intrinsic stability of the acetabular cup without the need for implantation of screws, so that less blood loss, shorter operative time.
