Objective: In my hospital Endocrinology 115 cases of type 2 diabetes as a research object, detection of serum cystatin C in type 2 diabetic nephropathy in early diagnosis and its clinical application of the relationship between the glomerular filtration rate. Methods: According to WHO1999 diagnosis and classification of diabetes standard choice of type 2 diabetes has been diagnosed 115 patients were male and 74 cases, 41 cases of female patients, aged 30-80 years old. When patients in the diet patterns of life, and counts the total 24h urine specimens, mix after taking l0ml urine albumin excretion send (UAE). The next morning after 10h of fasting venous blood and immediately inspected, measured cystatin C (CysC), total cholesterol (TCHO), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), fasting plasma glucose (FPG), blood urea nitrogen (BUN), serum creatinine (SCr), glycosylated hemoglobin (HbA1C), retinol-binding protein. Calculated using the simplified MDRD glomerular filtration rate (eGFR), CG formula to calculate creatinine clearance (CCr). 24h according to the measured results of urinary albumin excretion were divided into: 1, simple diabetic group (24-hour urinary albumin excretion lt; 30 mg); 2, early DN group (24-hour urinary albumin excretion volume = 30 ~ 300 mg); 3, clinical DN group ((24-hour urinary albumin excretion ≥ 300 mg). comparing the indicators at different levels of proteinuria detection of renal dysfunction proportion of different groups of proteinuria indicators to compare and analyze CysC, SCr, CCr correlation with eGFR. calculated by MDRD GFR, normal albuminuria group of patients will be divided into eGFR lt; 60ml/min/l.73m2 and eGFR ≥ 60ml/min/l.73m2 group, two groups of serum cystatin C and other indicators are compared and analyzed statistically. Results: 1, eGFR detection of renal dysfunction was significantly higher than CysC, SCr and CCr, the difference was statistically significant (p lt; 0.01), serum cystatin C and CCr detection rate considerably, there was no statistically difference (p gt; 0.05), the detection rate was significantly higher than both SCr and BUN , the difference was statistically significant (p lt; 0.01). especially in the normal albuminuria group, CCr and BUN detection rate is very low. 2, with the increase in the amount of urine albumin, eGFR, CCr, serum cystatin C , the amount of increase in serum creatinine. 3, CysC with eGFR a good correlation, r = -0.625, but the correlation is less than CCr and SCr. 4, in the 24-hour urinary albumin excretion lt; 30 mg in patients , eGFR lt; 60ml/min/l.73m2 patients cystatin C levels were significantly higher in patients with eGFR ≥ 60ml/min/l.73m2, the difference was statistically significant. Conclusions: Serum cystatin C and renal Small ball filtration rate have a good correlation, and in normal albuminuria in diabetic patients that can prompt early glomerular filtration rate decline.
