Abstract


Objective: thyroid cancer (thyroid carcinoma, TC) is more common human malignancy, accounting for about 1% of systemic cancer, the average annual incidence rate of about 2 to 4/100 000, the disease is more common in women, and the incidence of The upward trend in recent years. Pathological type of thyroid cancer, papillary carcinoma, follicular carcinoma, undifferentiated carcinoma and medullary carcinoma of the four types. Except medullary thyroid carcinoma, the vast majority are originated in thyroid follicular cells. The thyroid carcinoma Development is a complex process, role in a variety of steps from a variety of factors. Radiation injury, genetic, machine vivo gene changes are many factors that may be associated with thyroid cancer. CD44 protein is a cell surface transmembrane glycoprotein widely distributed in the body, and its main role is to generate heterogeneity adhesion, adhesion between the body's normal cells and stromal tumor cells, so that in play a role in promoting cancer invasion and metastasis. CD44 protein is located in the short arm of human chromosome 11, close to the size 50kb, is composed of 20 highly conserved exon and intron inserted into the middle of exon varying sizes will segregate them. Different transcription of CD44 protein 20 exon standard (s) and variant (v) can be divided into two broad categories. Expression of CD44v6 protein expression and the rate of malignant tissues of the same, the positive expression were more prone to local invasion and distant metastasis; positive expression than those with negative five-year survival rate, and poor prognosis. In recent years, a large number of research results show that CD44v6 protein plays an important role in the process of lymph node metastasis of thyroid cancer; positive expression can be seen as a predictor in clinical lymph node metastasis, and is an important indicator to assess patient prognosis. Mucin 1 (referred to as MUC1) is a high molecular weight glycosylated glycoproteins, are transmembrane molecules, and is also known to attach the membrane protein. Play a role in maintaining the integrity of the epithelial cells, update and differentiation, and the occurrence of cancer and metastasis, MUC1. MUC1 in normal epithelial tissue, to play an important protective role in the malignant cells, however, possible to reduce the adhesion between cancer cells to produce adhesion of cancer cells to normal cells. MUC1 by the steric hindrance of the tumor cells to evade the body's immune system to recognize and conducive to cancer metastasis. Therefore, the expression of MUC1 in the development of certain malignancies, transfer, play an important role in the treatment and prognosis, as some epithelial tumors occurrence of lymph node metastasis markers. This experiment to study the expression of CD44v6 in thyroid carcinoma and MUC1, to discuss their role and significance in the incidence of thyroid cancer, the development and transfer process, thus further reflected through these tumor markers of thyroid cancer exist to assist early diagnosis, develop treatment programs and evaluate the therapeutic effect as well as monitoring the prognosis of thyroid cancer patients. Methods: Using immunohistochemistry (Immunohistochemistry, IHC) SP method detected 75 cases of thyroid cancer (59 cases of papillary thyroid carcinoma, eight cases of follicular thyroid cancer, 2 cases of medullary thyroid carcinoma, six cases of thyroid undifferentiated carcinoma) and 16 cases of thyroid adenoma tissue paraffin sections organizations of CD44v6 and a MUC1 positive expression rate to analyze the relationship between the positive rate of thyroid cancer metastasis and invasion. Results: Expression of CD44v6 in thyroid tissue expression of CD44v6 protein positive expression rate of thyroid adenoma and thyroid cancer were 18.8% (3/16) and 62.7% (47/75), in thyroid cancer was significantly higher than the thyroid adenoma (χ 2 = 10.274, P lt; 0.05); positive expression rate of lymph node metastasis (23/28) was significantly higher than that without lymph node metastasis (24/47) (χ 2 = 7.244, P lt; 0.05); CD44v6 protein positive expression in thyroid cancer Ⅰ ~ Ⅳ staging was statistically significant (χ2 = 17.348, P lt; 0.05), the expression rates were 48.3%, 75.0%, 94.1% and 22.2%. 2 MUC1 positive expression of MUC1 protein expression in different thyroid tissue within the thyroid adenoma and thyroid cancer rates were 25.0% (4/16) and 70.1% (53/75), its expression in thyroid cancer higher than the thyroid adenoma (χ2 = 11.751, P lt; 0.05); Expression of MUC1 protein in there positive expression rate (24/28) in the lymph node metastasis was significantly higher than that without lymph node metastasis (29/47) (χ2 = 4.881 Ⅰ ~ Ⅳ thyroid cancer staging, P lt; 0.05);, during the positive expression rate is not statistically significant (χ2 = 7.391, P = 0.060 gt; 0.05), the expression rate was 62.0%, 80.0%, 88.2% and 44.4%, respectively. Conclusion 1 CD44v6 protein positive expression difference exists between thyroid benign and malignant tumors; associated with thyroid cancer, clinical stage and lymph node metastasis. 2 MUC1 protein positive expression differences between thyroid benign and malignant tumors may be associated with the incidence of thyroid cancer; thyroid cancer lymph node metastasis; clinical stage no significant relationship with thyroid cancer. 3 in thyroid cancer, the expression of CD44v6 protein expression of MUC1 protein has relevance; Both play an important role in the progression and metastasis of thyroid cancer. CD44v6 protein and MUC1 protein expression of good judgment thyroid tumor malignant and malignant degree of prediction of tumor invasion and metastasis may be present important clinical value, or licensed as an important biological indicators for evaluating prognosis.
