Comparison of Sevoflurane and Propofol in Patients Undergoing Aneurysm Clamping Operations

Abstract


Objective:To investigate the influence of anesthetized by different anesthetics in aneurysm clamping operations.Methods:Thirty-six adult patients of intracranial aneurysm (aged 28-67、ASAⅡ-Ⅲ、Hunt-Hess score grade 1-3,rule out diabetes mellitus、legs vascular disease、serious cardiopulmonary diseases、obvious high-ICP)scheduled for neurosurgery under general anesthesia were randomized to two qroups(n=18)according to different anesthetics.The patients of group A were anesthetized by sevoflurane,others used propofol.Ventilated patients with intermittent positive pressure and kept PaCO2 on 30-40mmHg by adjusting respiratory frequency according to blood gas analysis.Monitored invasive blood pressure and controled blood pressure(when operator clamping the aneurysm, kept blood pressure≈70% preoperative blood pressure,after that elevated blood pressure smoothly and steadily).Ranked patients by Short Orientation Memory Concentration Test(SOMCT) and Modified Rancho Los Amigos Scale(RLAS) before sugery and after two days,recorded the scores.Draw blood before sugery(T1),at the end of operation(T2),and after two days(T3),and tested NSE and MBP.Results:1:When clamping the aneurysm, patients’SBP and MAP of group A is lower than group B,the patients who used glyceryl trinitrate were fewer than group B (P<0.05).2:The difference of postoperative hospitalization time、the scores of SOMCT and RLAS、NSE and MBP in blood between two groups was not statistically significant (P> 0.05).Conclusion:1:Using sevoflurane could control blood pressure better than using propofol in aneurysm clamping operations.2:Two anesthetics hadno obvious difference in postoperative recovery and NSE、MBP of patients’blood.
