Significance of Intraoperative Indocyanine Green Fluorescence Videoagiography during Neurosurgical Procedure

Abstract


Ojective:To study the significance of indocyanine Green fluorescence videoangiography during neurosurgical proceducrs.Methods:During neurosurgical proceducre,43 patients underwent indocyanine green (ICG) fluorescence videoangiography.There were aneurysms in 24 cases,arterioverious malformation(AVM) in 4 cases.parasagittal sinus menigioma in 6 cases,large blood vessels in or surrounding tumors in 6 cases,and tumor with sufficient blood suplly in 3 cases.ICG fluorescdnce videogiography were performed before and after neurosurgical management of lesions in 43 patient.Results:The intraoperative ICG fluorescence videoangiography of 24 cases of aneurysm found parent artery stenosis in 3 cases, residual aneurysm neck in 2 cases,, distal branch stenosis in 2 cases,, By adjusting the position of aneurysm clip until satisfary blood flow was restored according to the finding of ICG fluorescence videoangiography, postoperative head CT scan proved that there was no ischemic infarction. AVM nidus, feeding arteries and draining veins could be found by ICG fluorescence angiography before resection of the AVM in 4 cases, ICG fluorescence videoangiography guide the resection of the AVM effectively. Postoperative DSA revealed that total resection of AVM nidus in 3 cases residual of nidus in 1 case which recovered well after receiving y-knife therapy. ICG fluorescence videoangiography could assess the blood flow of veinal sinus before or after removing the parasagittal sinus meningoma. It also could demonstrate the blood flow in vessels in or surrounding the tumors,and vasospasm happened after lesions resection in 4 cases. Blood stream recovered soon after the vessels were covered by papaverine cotton about 2 minutes.The tumor bourdaries,feeding arteries and draining veins could be well found by ICG fluorescence videoangiography before resection.Conclusion:ICG fluorescence videoangiography is an important method in improvement of curative effect of intracranial aneurysm and AVM. It also could provide an valuable imformation for assessment of blood flow of venial sinus and large blood vessels bfore and after resecting the lesions adjacent to the vessels.
