Abstract


Objective: To investigate the diagnosis of invasive pituitary adenomas and improved pterion treatment of invasive pituitary adenomas operative techniques and benefits. Methods: A retrospective analysis of our hospital in March 2008 to March 2009 period of surgical treatment of 60 cases of large or giant invasive pituitary adenoma patients , all patients diagnosed by preoperative MRI and a modified pterional the microscopic surgical resection, postoperative MRI review. Results: The group of 60 cases in micro-surgery , the full cut 59 cases ( 59 /60 , 98.4% ) , subtotal one case ( 1/60 , 1.6% ) . Subtotal resection in one case and the next person hypothalamus, optic nerve, optic chiasm , perforating artery close adhesion , only subtotal rows . Plus postoperative radiotherapy did inhibit growth . Common postoperative complication was transient hypopituitarism and diabetes insipidus . Conclusion: head CT and MRI in the diagnosis of pituitary adenomas has important value , improved Pterional 's micrographic surgery is the treatment of invasive pituitary adenomas effective and safe method of treatment. Based on tumor type, location and adjacent relationship , select the appropriate surgical approach and surgical clearance , fully reveal the optic chiasm , pituitary stalk and the bottom three rooms and other key surgical site ; intraoperative careful to distinguish between tumor and the hypothalamus - pituitary system of relations , sharp separation of adhesions between tumor and vital structures , skilled microsurgical technique is a key factor in successful treatment .
