BACKGROUND: Diabetic bladder disease (DCP) diabetes (DM) common chronic complications, about DM 40% ~ 80%. It was an insidious onset and asymptomatic progress, which is characterized by progressive loss of ability to urinate often to severe urinary retention and urinary tract infections and even renal failure, was valued by clinicians, the prognosis is poor. For patients with previous DCP judgment of the lower urinary tract function is mainly based on patient self-reported symptoms of dysuria, lack of objective diagnostic indicators. Mastery in the progression of the disease and determine treatment methods have some limitations. Application of urodynamic testing indicators measured changes in urodynamic diagnosis of bladder voiding dysfunction caused by disease has an extremely important significance. Urodynamic bladder function is the most important means of objective evaluation. Urodynamic indicators may reflect changes in bladder function in patients with DM. Therefore used to determine whether a patient suffering from DM DCP. And the recent view is that such use urodynamics for early diagnosis. DCP if you can get early diagnosis and aggressive treatment of part of its course is reversible. Current clinical applications in more by urologists invasive, comprehensive urodynamic testing methods, mainly to themselves apparent difficulty urinating DCP patient testing, there has been significant clinical urinary retention and the need for detrusor pressure was measured to determine whether DCP must bladder fistula surgery patients. DCP for no apparent difficulty urinating patients clinically difficult to implement. Therefore, the loss of early detection, diagnosis timing. Non-invasive urodynamic method is a recently proposed concept, the advantage is non-invasive urodynamic testing indicators. DCP can be detected early. DCP study is a cross-disciplinary research, has not been reported with noninvasive early detection DCP urodynamic study. Objective: To evaluate noninvasive method urodynamic urinary tract symptoms in patients with DM without urodynamic testing indicators to early detection of DCP. DCP for the early prevention and treatment provide the basis. Methods: 70 patients with type 2 DM patients and 30 non-DM group were no indicators of urodynamic testing, the DCP group and DM group and the control group were compared maximum urinary flow rate, the average rate of urination, bladder capacity and beginning to urinate residual urine volume and other non-invasive urodynamic indices. According to the limit of 5 years duration of DM compare these indicators. Results: All 70 cases of patients with DM bladder residual urine in 34 cases, with an average residual urine volume 30.1 ± 27.1ml (7 ~ 139ml), DCP detection rate of 48.6%. DM group of 36 patients and normal control group of 30 patients had no residual urine volume. Compared with normal control group, DCP group and the DM group maximum flow rate, average flow rate was significantly lower (P <0.01). DCP group compared with DM group these indicators have gradually decrease. More than 5 years duration of DM group and the duration of less than five years set maximum flow rate, average voiding rate decreased with the duration of the extension, the second group and the normal control group were significantly lower (P <0.01). Conclusions: (1) DM patients with maximum flow rate reduction and the emergence of residual urine can be used as early detection and diagnosis DCP screening targets. (2) non-invasive urodynamics is simple, easy to implement. Can be used as early detection and diagnosis DCP screening method. It can be widely used with the early detection of the DCP features.
