Purpose of lamivudine (LAM) as control , evaluate the efficacy of adefovir dipivoxil (ADV) treatment of adults with chronic hepatitis B (CBH) the effectiveness and safety. Methods 47 cases of adult patients with chronic hepatitis B were randomly divided into groups and LAM ADV group . ADV group received adefovir dipivoxil 10mg, 1 times / day , LAM group received lamivudine 100mg, 1 times / day , course of treatment was 52 weeks . In the follow-up period, patients were collected serum ALT, HBV DNA quantification , HBV markers and other projects inspection, 52 weekends , the application of fluorescent PCR to detect LAM patients YMDD mutation of HBV DNA . Results The treatment group was 52 weekend ADV 52.17%, LAM group was 69.57% (P> 0.05), the two groups have similar efficiency and HBeAg-negative patients in both groups were higher than the effective HBeAg-positive patients (P <0.05) ; LAM group in the initial treatment of HBV DNA negative conversion rate and ALT normalization rates were higher than ADV group , but with medication time, the two negative rate of HBV DNA and ALT normalization rate gradually approaching ; overall recurrence rate and HBeAg -positive patients with LAM group, the recurrence rate is high , HBeAg -negative patients with ADV group, the recurrence rate is high, there was no statistically significant difference (P> 0.05); groups overall adverse reaction rate was 12.5% ??(ADV) and 17.39 % (LAM), no significant difference (P> 0.05), there were no serious adverse events ; 23 cases of LAM group, 3 patients YMDD mutation , which YIDD 2例, YVDD1 cases , 13% of the total variance , 2 cases accompanied by significantly higher HBV DNA and ALT levels titer anomalies . Conclusion Domestic adefovir dipivoxil and lamivudine in the treatment of adults with chronic hepatitis B is similar efficacy and safety of 52 weeks effective ; adefovir dipivoxil slow onset compared with lamivudine , with the medication time, the efficacy of both gradually approaching ; LAM patients treated with YMDD mutations occur in 52 weekends , 13% of the total variance .
