Abstract


Objective: To investigate the jejunal mucosal structure of different age groups and enteral nutrition tolerance relationship . Methods: Jiangxi Medical College Affiliated Hospital of General Surgery, the Second Division from January 2004 to March 2005 line radical gastrectomy 30 patients . Divided into two groups according to age , the younger age group ( <40 years ) in 14 cases, 16 cases of old age group ( > 65 years ) . Surgery for full-thickness jejunal mucosal line observed by light microscopy and image analyzer to measure the jejunal mucosal thickness , villus height and width of change . By transmission electron microscopy observation of the arrangement of the microvilli of jejunal epithelial cells , the level and structure of the absorption cell mitochondria change . Patients in each group after the first day intravenous rehydration therapy , after the second day began with 100 ordinary exercise enteral nutritional support and fluid therapy were postoperative enteral nutrition in patients tolerated , such as abdominal pain , bloating, diarrhea , nausea, vomiting, dumping syndrome occurred . The incidence of complications were compared between groups ; villi height, width, thickness of the mucosa were expressed as mean standard deviation between the two groups and old age group comparison between patients with or without abdominal distension and diarrhea . Application SPSS 13.0 for Windows statistical package handling , measurement data with the Cochran Cox approximate t-test , the complication rate compared using Fisher's exact test of the 2 × 2 tables . Results: 1. Elderly group of patients with postoperative enteral nutrition , abdominal pain, bloating, diarrhea was significantly higher in the younger age group, P < 0.01 ; elderly group of patients than the younger age group of patients with jejunal villi short , width wider , mucosal change thin P < 0.05 ; ultrastructure observation of the younger age group of patients jejunum microvilli neat and compact, the length of the same ; elderly group microvilli irregular , sparse , of varying lengths . Normal young cells , old age group, swelling of mitochondria cristae dissolved , even vacuolization structural changes .
