Clinical Significance of Lymph Nodes Micrometastases in Stage Ⅱ Rectal Cancer Patients

Abstract


Objective To investigate the Phase II rectal cancer patients with lymph node micrometastasis detection of clinical significance . Total mesorectal excision line on January 2000 to August 2001 42 patients with rectal II specimens 484 lymph nodes intermittent biopsy combined cytokeratin-20 (CK-20) immunohistochemistry staining , according to the follow-up data the literature analysis II rectal cancer patients with mesenteric lymph node micrometastasis clinical significance . Results The average per patient with lymph node 11.5 micrometastases occur total number of cases to 15 cases , the incidence was 35.7% ( 15 / 42 ) , lymph node micrometastases detected a total of 33 , the detection rate was 6.82% ( 33 / 484 ). Follow - up of 12 to 80 months , 60 months , 32 patients survived , and 11 deaths . Lymph node micrometastasis group of five-year survival rate was 40.0 % , lymph node micrometastasis negative group, the five - year survival rate was 92.6% . Survival rates between the two groups by the Log-rank test was significantly (P <0.05). Cox regression analysis shows lymph node micrometastasis RR value of 11.435 , only the introduction of the risk factors , closely linked with the recurrence and metastasis . Conclusion lymph node micrometastasis important prognostic indicator of Phase II cancer patients , lymph node micrometastasis in this study is an independent prognostic indicator of metastasis and recurrence of rectal cancer patients , lymph node micrometastasis of rectal cancer after surgery patients may benefit from chemotherapy .
