Clinical Significance of eIF4E and CyclinD1 Expression in Primary Colorectal Cancer

Abstract


Objective:To explore clinical significance of expression of eukaryotic translation initiation factor 4E (eIF4E) and CyclinD1 proteins in the tumorigenesis and progression of colorectal carcinoma.Materials and MethodsThe specimens involved had been collected from the Pathology Department of the Zhejiang Cancer Hospital, including 80 cases of primary colorectal cancer (CRC) surgically resected,20 cases of normal mucosa and 30 cases of colorectal adenoma. None of the patients with colorectal cancer had received any preoperative therapy. Standard two-step EnVision method of immunohistochemical staining was performed to detect the expression of eukaryotic translation initiation factor 4E (eIF4E) and CyclinD1 proteins. Data were analyzed with chi-square, Fisher’s exact probability test, spearman test and Kaplan-Meier analysis by SPSS 13.0 statistical package.ResultThe positive rates of eIF4E expression in CRC, colorectal adenoma and normal mucosa were 10%(2/20),83.3%%(25/30),92.5%(74/80), respectively. The difference of elF4E expression between CRC and normal mucosa groups was statistically significant (P<0.01).The positive rates of CyclinD1 expression in CRC, colorectal adenoma and normal mucosa were 0%(0/20),46.7%%(14/30),55%(44/80), respectively. The difference of cyclinDl expression between CRC and normal mucosa groups was statistically significant (P<0.01).eIF4E protein expression in CRC was closely correlated with infiltration depth, lymph node metastasis, TNM stage and 5-year survival (P<0.05), whilst unrelated with gender, age, tumor location, tumor size, histological grading, tumor embolus (P>0.05).CyclinDl protein expression in CRC was closely correlated with histological grading (P<0.05), lymph node metastasis and TNM stage (P<0.01), whilst unrelated with other clinicopathologic characteristics.There was a positive relationship between the expression of eIF4E and CyclinD1 in CRC (r=0.914, P<0.01).Conclusion1. The abnormality of eIF4E expression may be involved in process from adenoma to progressive adenocarcinoma of colorectum, which indicates that eIF4E might play an important role in the tumorigenesis of CRC.2. CRC with higher expression of eIF4E protein is characterized by deeper infiltration, more lymph node metastasis and worse prognosis, which suggests that eIF4E could be a useful prognostic marker in CRC.3. elF4E expression in CRC is positively correlated with CyclinD1 expression, which suggests there is a coordinate action between them.
