Abstract


Objectives To evaluate the effect of photodynamic therapy (PDT) which toprevent the recurrence and progression of non-muscle invasive bladder cancer afterTransurethral Resection of Bladder Tumor (TURBT),and compare the role betweentwo generation photosensitizers.Patients Select138patients as experimental group whom were diagnosed asnon-muscle invasive bladder cancer by using cystoscopy and biopsy in our hospitalfrom June2005to April2011. Group A had47patients which usedhematoporphyrin as photosensitizer before August2007. And Group B consisted of91patients which used5-Aminolaevulinic Acid (5-ALA) as photosensitizer afterAugust2007. Controlled group were138patients which using conventionaltreatment.Methods Experimental groups (A+B) and Controlled group all wereadministered a immediate intravesical pharmacotherapy within24h after TURBT.And Experimental groups (A+B) were performed PDT within one week afterTURBT. After administration of photosensitizer,group A was performed PDTs in36h and60h, then intravenous drip photosensitizer100mg at72h after firstadministration.12hours later, Group A was performed third PDT. Each timecontinued20min. Group B did not have PDT until they have administration of5-ALA before2h. Group B performed PDT only once,and time was60min. Bothexperimental groups received whole bladder irradiation using semiconductor laser at wavelength of630nm,with a power density of50J/cm2approximately. Afterdischarged,all the patients routinely received intravesical pharmacotherapy oneyear.Results In experimental groups,the total rate of tumor recurrence was33.3%(46/138). The rate of tumor recurrence in Group A and B respectively was31.9%(15/47) and34.1%(31/91).15patients accounting for the10.9%(15/138) ofexperimental group (A+B) progressed to muscle invasive bladder cancer. Group Awas10.6%(5/47) and group B was11.0%(10/91). In controlled group,The rates oftumor recurrence and progression were50.7%(70/138)and26.1%(36/138). Theresults showed, patients in experimental group (A+B) had less rates of tumorrecurrence and progression than controlled group (P<0.05). Either group A or Balso had less rates of tumor recurrence and progression than controlled group(P<0.05). But there was no significant difference of the tumor recurrent rate andprogressive rate between group A and B (P>0.05).Conclusion On the basis of traditional treatment combined with PDT,maydecrease the rates of tumor recurrence and progression of non-muscle invasivebladder cancer. But the effects of treatment are no significant difference betweentwo generation photosensitizers.
