Sufentanil and Remifentanil in Tracheal Intubation Stress Response Influence

Abstract


[Objective] To observe the rapid induction of a single intravenous bolus injection of small doses of fentanyl, sufentanil, remifentanil compound the same sedative antipsychotic drugs hemodynamic changes before and after endotracheal intubation and stress hormones changes. Discussion reduce cannula stress response effects Preferably, the safety of the drug in three groups of drugs. [Methods] 60 patients undergoing elective anesthesia (abdominal surgery) in patients with ASA (American the anesthetic Association) condition grading Ⅰ ~ Ⅱ grade, male 17 cases, female 43 cases. Age 18-50 years old, unintentional lungs and other vital organs brain lesions, endocrine function abnormalities, difficult intubation. Were randomly divided into three groups: the fentanyl group (F), sufentanil group (SF group), remifentanil (RF group). 20 patients in each three groups first followed by intravenous midazolam (0.1mg/kg), vecuronium (0.15mg/kg), propofol (2mg/kg). Finally intravenous analgesics: F group intravenous fentanyl 3μg/kg, the the SF groups vein injection of sufentanil 0.5μg/kg, the RF group intravenous remifentanil 1μg/kg, induction time 3min time line laryngoscope endotracheal intubation under direct vision. Record of burglary after 5 minutes of supine - basic values ??(t 0 ), before intubation immediate (t 1 ), after intubation 1min (t 2 ), after intubation 3min (t 3 ) after intubation 5min (t 4 ) after intubation 7min (t 5 ), intubation after 10min (t 6 ) time points blood pressure, systolic blood pressure (SBP), heart rate (HR). And t 1 t 3 t 4 t 6 point in time each venous blood, separation plasma, measured using radioimmunoassay of cortisol (Cor.) and aldosterone (ALD) levels in the blood. [Results] (1) the three groups intubation before and after SBP changes within the group, with t 0 comparison: t to 1 t 5 , t < sub> 6 point-in-time group SBP decreased significantly (P <0.05); 2 point-in-time Group F the SBP significantly higher (P <0.05), SF group SBP decreased significantly (P <0.05), the RF group SBP was no significant change (P> 0.05); 3 t 4 point in time SF RF group SBP decreased significantly (P <0.05), The F Group SBP no significant change (P> 0.05). Between the two groups: SF and F group, t 2 t 3 t 4 t 5 t 6 point-in-time SBP was significantly lower than the F group (P <0.05); RF and F group, t 1 t 2 t 4 point-in-time SBP was significantly lower than the F group (P <0.05). (2) changes in the HR group before and after the three groups intubation and t 0 comparison: t to 1 t 3 point in time three groups of HR change not significant (P> 0.05); t 2 point in time F RF group HR was significantly higher (P <0.05), SF group HR no significant difference (P> 0.05); t 4 t 5 t 6 point in time SF group HR was significantly lower (P <0.05), F group, the RF group HR no significant change (P> 0.05 ). Between the two groups: t 4 t 5 t 6 4 , t 6 point-in-time RF group had no significant difference (P> 0.05); SF group with RF group t the the 1 t < sub> 2 t 3 t 4 t 5 t 6 point in time SF The group HR significantly lower than the RF group (P <0.05). Changes within the group of (3) three sets of intubation before and after Cor. 1 point-in-time comparison with t: t three groups Cor 3 point in time was significantly higher (P <0.05 ); t 4 point-in-time Group F Cor significantly higher (P <0.05), SF, RF group Cor significant change (P> 0.05); t 6 Group F Cor point in time. significantly higher (P <0.05), the SF groups Cor. significantly lower (P <0.05), RF group Cor. was no significant change (P> 0.05). Between the two groups: Group F, t 3 t 4 t 6 point in time SF, RF Group Cor. Significantly lower than Group F (P <0.05). (4) the three groups intubation before and after the change of ALD group with t 1 point-in-time comparison: t 3 0.05); t 4 point-in-time group ALD no significant difference (P> 0.05); of t 6 point in time RF group ALD significantly lower (P <0.05), F, SF group ALD no significant change (P> 0.05). Between the two groups: three groups of ALD t 3 point in time, no significant difference (P> 0.05), t 4 t 6 point in time SF The RF group ALD was significantly lower than the F group (P <0.05). [Conclusion] Compared with fentanyl, sufentanil and remifentanil suppression intubation hemodynamic changes effect better. 2 compared with fentanyl, sufentanil and remifentanil reduce intubation stress hormone levels better.
