Abstract


Objective: Renal cell cancer is highly vascularized malignant neoplasms and is one of Urinary system common substantive tumor, which Ranked second in urinary tract tumors. The early symptoms of kidney cancer are not obvious. Most of the patients with renal cell cancer have already belonged to middle-late stage when seeing a doctor. Renal cell carcinoma’s transfer rate is higher. The main transfer way is lymph metastasis and hematogenous metastasis. However, the current treatment of RCC is radical surgery, but the rate of recurrence is very higher. And the results of radiotheraphy and chemotherapy are poorer. Urgent needing other effective treatment methods . Along with the development of molecular biology in recent years,A large quantity of factors associated renal cell carcinoma’s growth and progress were found. Which have provided the theory basis and the molecular foundation for the kidney cancer’s treatment. But the occurrence、development and metastasis are extremely complex dynamic process, Which involved many tumor-associated factors participating and controlling,and the pathogenesis is not clear. Vascular endothelial growth factor is an important angiogenesis positively regulating factor. which is closely related with the development and metastasis of renal cell carcinoma, and is one of the target of anti-cancer therapy. chemokine receptor(CXCR4)is the only receptor of Stromal cell derived factor-1(SDF-1), Studies suggest that CXCR4 act as an important role in rumor-specific metastasis. CXCR4 and VEGF were reported to correlated with malignant tumors such as cholangiocarcinoma,colon cancer,gastric cancer,esophageal cancer and so on, but in renal cell carcinoma which are in studing in domestic and overseas. Our experiment mainly discusses vascular endothelial growth factor (VEGF) and chemotactic factors receptor (CXCR4) in renal cell carcinoma’s expression and the clinical significance. Which may be provide a theoretical basis of diagnosing and treating for RCC.Methods: Collect 56 patients hospitalized patients with kidney cancer resection specimen,Including 20 cases of kidney cancer specimen with lymph node metastasis,and randomly select 10 cases of normal renal tissue from 56 cases. All specimens were fixed in formalin and they had been confirmed by pathology. 56 cases were not received radiotherapy and chemotherapy treatment before surgery. The expression of CXCR4 and VEGF was checked in 56 cases of RCC、20 cases of kidney cancer with lymph node metastasis and 10 cases of normal renal tissues by immunohistochemical technique from protein level. And the relations between them and the kidney cancer’s clinical pathology graduation、stage lymphatic metastasis ,age, sex and tumor size were analyzed。Results: The positive rates of CXCR4 and VEGF proteins were 66.1%(37/56)and 73.2%(41/56)repectively,which were much higher than those in normal tissues(20% and 30%, respectively),The differences had remarkable statistically significant(P<0.05),According to the TNM staging of 2002 AJCC, In theⅠ、Ⅱ、Ⅲ、Ⅳstaging of 56 cases of renal cell carcinoma the positive rates of CXCR4 were 42.9%, 59.1%, 83.3%, 100.0% respectively. The differences had remarkable statistically significant(P=0.023),The positive rates of VEGF were 50.0%,68.2%,91.7%,100.0%respectively,The differences had remarkable statistically significant(P=0.027)。Their percentage elevates along with the increase of pathology graduation。The positive rates of CXCR4 and VEGF proteins in Renal cell carcinoma specimens with lymph node metastasis were 85.0%,90.0%respectively,which were much higher than those in Renal cell carcinoma specimens without lymph node metastasis(55.6%,63.9%respectively),The differences had remarkable statistically significant(P<0.05),The expression of CXCR4 protein was significantly positively correlated with that of VEGF protein(r=0.315,P<0.05) in Renal cell carcinoma. The expression of CXCR4 and VEGF was closely relate to stages of tumor, pathological classification, lymphatic metastasis, degree of invasion, and MVD。But was not related to sex、age、Pathologic types、Tumor size of the patient。 Conclusion: CXCR4 and VEGF are highly expressed in renal cell carcinoma,their positive rate elevates along with the increase of pathology graduation. Especially in renal cell carcinoma with lymph node metastasis specimens the positive rate are especially remarkable. and in RCC their expression were positively correlated. Which point out CXCR4 and VEGF promote mutually in the kidney cell cancer, promotes the kidney cancer’s multiplication, development and shift together。CXCR4 and VEGF can be used as important indicators in judging the metastasis prognosis of renal cell carcinoma,which also offer prospects for the treatment of renal cell carcinoma。
